Physical Therapist (PT) – Day Shift (Urgent Need)

Contract Duration: 13 Weeks

Schedule: Tues–Sat or Sun–Thu

Shift: 8:00 AM – 4:30 PM

Position Summary

We are urgently seeking a skilled and compassionate Physical Therapist (PT) for a 13-week contract assignment at a skilled nursing/rehabilitation facility. The PT will evaluate, plan, and provide evidence-based therapeutic interventions to help residents restore functional mobility, reduce pain, and achieve optimal physical performance.

Key Responsibilities
  • Conduct comprehensive physical therapy evaluations and develop individualized treatment plans.
  • Provide therapeutic interventions focused on mobility, strength, balance, gait training, and functional independence.
  • Use therapeutic exercises, modalities, manual therapy, and equipment to increase patient function.
  • Document all evaluations, daily treatment notes, progress reports, and discharge summaries accurately and timely.
  • Collaborate closely with the interdisciplinary care team including OT, SLP, nursing, and case management.
  • Monitor patient progress and adjust treatment plans as needed.
  • Educate patients and families on home exercise programs, safety techniques, and functional training.
  • Ensure compliance with all state, federal, and facility guidelines.
  • Participate in care conferences and clinical meetings as required.
Qualifications
  • Active Physical Therapist (PT) license in the state of assignment (or ability to obtain).
  • Graduate of an accredited DPT or MPT program.
  • CPR/BLS certification, as required.
  • Prior experience in skilled nursing, long-term care, geriatrics, or rehab strongly preferred.
  • Strong clinical assessment, treatment, and documentation skills.
  • Ability to commit to the full contract duration and scheduled workdays.
