SUMMARY:
The Medical Respite Program Coordinator, a Licensed Clinical Social Worker (LCSW), reports to the Director of Discharge Planning or their designee and is responsible for overseeing the planning, direction, and daily operations of Brown University Health's Medical Respite Program. This role provides care support and case management for high-risk patients requiring medical respite who lack stable housing. The LCSW evaluates, diagnoses, and treats a broad range of psychosocial issues, using clinical expertise to help patients and families address challenges stemming from mental or physical illness, while adhering to the NASW Code of Ethics and professional standards of clinical social work practice.

RESPONSIBILITIES:
Brown University Health employees are expected to successfully role model the organization's values of Compassion, Accountability, Respect, and Excellence as these guide our everyday actions with patients, customers and one another. Manages the ongoing analysis, development, and implementation of the Medical Respite Program to improve overall efficiency and effectiveness. Completes bio-psychosocial evaluations to assess program eligibility and eliminate barriers to treatment, discharge and enhance the patient's health status and improve utilization of resources. Develops treatment plans and initiates clinical intervention, utilizing individual and group treatment modalities, and analyzes course and result of treatment to maximize health status and increase health care decision making. Documents all social work activity in the patients' medical records. Works collaboratively with patients and various members of interdisciplinary teams both in the hospital and community. Patient population may include those with, but not be limited to, the following concerns: medical complexities, homelessness, substance use, mental health, uninsured or underinsured, and other psychosocial needs. Works collaboratively and effectively with external stakeholders (such as community partners and payors) as needed to ensure the effectiveness of the program and that patients are provided with the appropriate support needed to engage with resources and improve their overall health and well-being. Provides education to assist patients and families in understanding, accepting and following medical and social work recommendations designed to restore the patient to optimum functioning within their capacity. Assesses patient and family advocacy needs, and coordinates appropriate advocacy activities within the hospital and in the community with an emphasis on self-determination for the client system with the aim at improving social support and community resources. Supervises and provides guidance to the Medical Respite Community Health Workers providing direct case management to the patients throughout their admission. As relevant to clinical assignment, remains current on available community resources, and accesses them as needed. Maintains and enhances professional expertise through appropriate educational activities. May attend training for specific areas of clinical responsibility. Participates in quality assurance activities in accordance to department, hospital, and The Joint Commission standards. Adheres to Brown University Health, Hospital, and department policies and procedures to maintain a safe environment of care and be in compliance with all laws, regulations and standards. Performs other job-related duties as assigned.
MINIMUM QUALIFICATIONS:
EDUCATION:
Masters Degree in Social Work from an accredited graduate school of Social Work and a current license from State of RI, Board of Social Work Licensure. Minimum requirement for licensure is Licensed Clinical Social Work level (LCSW). When Social Worker is eligible (two years or 2000 hours of post masters supervised practice) it is mandatory that they obtain Licensed Independent Clinical Social Work (LICSW) status. Knowledge of health care and health care delivery systems. Analytical skills are necessary to evaluate patients' concrete needs and to formulate and implement a treatment plan. Interpersonal skills to effectively interact with patients, families, medical staff, and outside agencies in providing services.
EXPERIENCE: At least two to three years of professional experience in a healthcare setting, human services, or related field, preferably working with individuals experiencing homelessness or underserved populations. Must exhibit strong interpersonal skills as well as a collaborative approach and style of communication in order to interact successfully on a daily basis with a wide and diverse population of health care providers, community agencies, patients and their families. Must demonstrate knowledge and skill necessary to provide care to patients throughout the life span, with consideration of aging processes, human development stages and cultural patterns in each step of the care process. A basic proficiency in the use of Microsoft office software programs including email, Outlook calendar and basic keyboard skills are also required. WORK ENVIRONMENT AND
PHYSICAL REQUIREMENTS: General hospital environment with occasional stressful conditions associated with patient care. Community and home visits required. Must be able to make hospital rounds through various patient care areas either by walking or through some other mobile means. Must be able to lift and or carry up to 10 lbs. to transport items from one patient care unit to the next. Must maintain an active driver's license.
INDEPENDENT ACTION: Performs independently within the department's policies and practices. Refers specific complex problems to the manager or designee when clarification of the departmental policies and procedures is required.
SUPERVISORY RESPONSIBILITY: Supervision of up to two employees. Supervision includes, but is not limited to, assigning work, scheduling, providing guidance and counsel and annual performance evaluations

$60,211.42-$99,331.96
