Job Summary

We are seeking a passionate and dedicated Home Health Occupational Therapist to join our dynamic healthcare team. In this vital role, you will assess, plan, and implement personalized therapy programs to help patients regain independence and improve their quality of life within the comfort of their homes. Your expertise will support a diverse patient population, including pediatrics, geriatrics, individuals with disabilities, and those recovering from acute or post-acute care episodes. This position offers an exciting opportunity to make a meaningful impact by delivering compassionate care in a home setting while collaborating with multidisciplinary teams to ensure optimal patient outcomes.

Responsibilities

  • Conduct comprehensive evaluations of patients' functional abilities, including assisting with activities of daily living (ADLs) such as dressing, grooming, and mobility.
  • Develop and implement individualized care plans tailored to each patient's needs, goals, and medical conditions.
  • Provide hands-on therapy interventions to improve motor skills, coordination, strength, and sensory processing for diverse populations including children with autism or those in early intervention programs.
  • Assist patients with discharge planning by coordinating services and educating families or caregivers on therapeutic exercises and safety strategies.
  • Collaborate closely with case management teams to monitor progress, adjust treatment plans, and ensure seamless continuity of care across inpatient, outpatient, skilled nursing facilities, or home health settings.
  • Document patient progress accurately in compliance with healthcare regulations and maintain detailed records of therapy sessions.
  • Educate patients and their families about adaptive techniques, equipment use, and strategies to promote independence at home or in community settings.

Qualifications

  • Valid occupational therapy license in the state of practice.
  • Demonstrated experience working with people with disabilities, autism spectrum disorder, geriatrics, or pediatric populations such as infants in early intervention or school-aged children.
  • Strong knowledge of physiology, anatomy, medical terminology, and functional anatomy relevant to occupational therapy interventions.
  • Experience providing care in various settings including inpatient hospitals (ICU experience preferred), outpatient clinics, skilled nursing facilities, post-acute care environments, or home health services.
  • Familiarity with developing care plans that incorporate case management principles and discharge planning strategies.
  • Ability to work independently while collaborating effectively with multidisciplinary teams including speech therapists and other healthcare professionals.
  • Excellent communication skills to educate patients and caregivers about therapy techniques and safety measures.
  • Knowledge of assistive devices and adaptive equipment to support patient independence in activities of daily living.
