Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) with Trach and Respiratory certification to join our dynamic healthcare team. In this vital role, you will provide expert nursing care to patients with complex respiratory needs, including those requiring tracheostomy management and ventilator support. We are specifically looking for someone to standby at the CP center with a patient in case she needs trach suctioning on M/W/F from 8:30am-3:30pm. Additional hours with other patients may be available as well.

Duties

  • Deliver comprehensive patient care by performing physical examinations, vital signs assessment, and detailed patient evaluations.
  • Manage tracheostomy tubes, ventilators, and airway devices with precision, ensuring proper function and safety.
  • Administer medications via injections, IV infusions, and other routes while adhering to strict protocols and safety standards.
  • Conduct medical documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic, Cerner, or Athenahealth to ensure accurate record-keeping.
  • Assist with diagnostic evaluations including specimen collection, processing, and basic medical imaging procedures.
  • Provide specialized care for patients with Alzheimer's disease, dementia, memory impairments, or behavioral health conditions.
  • Support complex cases involving acute pain management, infection control, hospice care, and post-acute recovery efforts.
  • Collaborate with multidisciplinary teams on discharge planning, case management, and patient education to promote health coaching and community-based care.

Experience

  • Proven experience as an LPN [performing respiratory care in diverse healthcare settings such as hospitals (including Level I Trauma Centers), nursing homes, outpatient clinics, and home health.
  • Critical care experience including ICU or emergency room environments is highly desirable.
  • Familiarity with advanced medical procedures such as catheterization, tube feeding management, spinal taps, and sterile techniques.
  • Knowledge of ICD-9/10 coding systems for accurate medical documentation and billing processes.
  • Experience working with people with disabilities or developmental delays in settings like assisted living or home & community care is preferred.
  • Prior exposure to infectious disease protocols, hospice care standards, or geriatrics enhances your ability to deliver specialized patient-centered services.
  • Competence in using EMR/EHR systems such as Epic or Cerner for efficient documentation and communication within healthcare teams.
