Job Purpose
The Inpatient Care Coordinator is responsible for supporting the Case management team. The coordinator will work directly with the Case Managers in day-to-day operations. This position is ideal for someone who is a strong communicator and team player.
Duties and responsibilities
- Check emails for notifications about inpatient admissions and/or discharges throughout the day.
- Check the fax queue and input authorization requests for home health, DME, throughout the day. Verify that attach documents belong to the appropriate case and the case dates.
- Answer calls for the department.
- Place outbound calls to members, PCP, hospital and SNFs.
- Utilize the Portal "Census" report to contact the facilities to see if members were discharged. Document all conversations to include date, time and to whom you spoke to in the ezcap case.
- Follow the inpatient case process as outlined Job Aid CM 407B.
- Requests clinical records for the admission and makes three attempts on three separate days to get clinical records, discharge summaries and orders, etc.
- Uploads and labels all clinical records, face sheets, discharge summaries and orders and place them in each member's case in Ezcap.
- Call PCP offices to set up post hospital follow up on visits and notify members of such.
- Utilizes various teams chats to communicate information to other team members on a timely basis.
- Checks team chats for orders that require authorization.
- Enter authorizations for DME, home care, etc. as requested by the case managers.
- Audit work in Ezcap and update any errors or changes as needed.
- Respond to requests for data integrity requests within the time frame outlined by the requestor.
- Check Point Click Daily Census for admits and discharges and enter into Ezcap.
- Download Point Click daily and weekend reports and enter admits and discharges into Ezap
- Download Point Click information (to include face sheet) if applicable and put in the member's case in Ezcap.
- Manage voice mail and respond to messages within 24 hours.
- Assist the inpatient case manager(s) with tasks (for example finding vendors, calling vendors to verify if they can accept the member, entering authorizations as needed) and other tasks as assigned.
- Reviews the Division of Financial Responsibility (DOFR) to assist with finding vendors.
- Maintain PHI and compliance with the Health Insurance Portability and Accountability Act (HIPAA).
- Proactively takes action to solve problems.
- Other duties as needed.
Qualifications
- 2 years healthcare administration experience
- 3 years customer service experience
- HMO/Medical Group experience preferred
- Must be able to work 8 - 5 PM Pacific Standard Time.
- Bilingual proficiency preferred
- Proficiency using Outlook, Microsoft Teams, Zoom, Microsoft Office (including Word and Excel) and Adobe
- Strong ability to multi-task and work in a fast-paced environment
- Strong ability in problem-solving
- Ability to work in an extremely confidential environment
- Strong written and verbal communication skills
- Technology savvy and highly organized
