They will also manage the discharge process and patient follow-up once the patient returns home. In addition, they will establish a strong sales presence with target audiences, including but not limited to: Acute Care providers, physicians, and others as identified within service area to promote awareness of the services for which the Community providesQualifications: Minimal 18 years of age with High School diploma or equivalentLPN, RN, BSW, MSW or Therapist (PT, OT, ST< PTA or COTA) experience desiredCase Management experience preferredAbility to communicate effectively with residents, families, visitors and co-workersAdhering to HIPAA regulations, OSHA regulations and state and federal guidelines.Ability to follow supervisor's instructions in a safe and adequate mannerExperience in skilled care or residential services for the elderly desired
