Role Description

This is a full-time hybrid role for a Case Manager/parent educator based in Chicago, IL, with opportunities for remote work. The Case Manager will be responsible for providing case management services, assessing client needs, and coordinating resources for families and youth. The role includes maintaining thorough documentation, monitoring client progress, and working collaboratively with organizational staff to address community challenges. The role will also require providing parenting education and child development assessments and support to 15 families with children ages 0-3 years old.

Qualifications

  • Strong case management skills
  • Excellent communication, active listening, and interpersonal skills
  • Organizational and time-management skills to handle multiple caseloads
  • Experience in resource coordination and advocacy within community networks
  • Familiarity with crisis intervention strategies and family support services
  • Bachelor's degree in Social Work, Psychology, Education or a related field
  • Knowledge of Chicago community resources and social service systems
