As a Nurse Care Manager, you will work in a partial care setting with positions located throughout Southern New Jersey. 

Responsibilities: 

  • Provide health education to clients in individual and group setting
  • Monitor prescriber scheduling with specific attention to provider productivity
  • Review of lab work and coordination with prescriber, as necessary
  • Documentation of all nursing activities provided
  • Support clients through nursing case management supports
  • Support prescribers with clerical support
  • Administer medications to include inject-able medications while maintaining precise medication records
  • Provision of emergency medical care to clients until emergency services arrive, when necessary
  • Coordination of client care with internal and external providers
  • Ability to work independently and as part of a team
  • Other duties as assigned

Qualifications:

  • High School Diploma or equivalent required;
  • LPN;
  • Valid Driver's License.
