As a Nurse Care Manager, you will work in a partial care setting with positions located throughout Southern New Jersey.
Responsibilities:
- Provide health education to clients in individual and group setting
- Monitor prescriber scheduling with specific attention to provider productivity
- Review of lab work and coordination with prescriber, as necessary
- Documentation of all nursing activities provided
- Support clients through nursing case management supports
- Support prescribers with clerical support
- Administer medications to include inject-able medications while maintaining precise medication records
- Provision of emergency medical care to clients until emergency services arrive, when necessary
- Coordination of client care with internal and external providers
- Ability to work independently and as part of a team
- Other duties as assigned

Qualifications:
- High School Diploma or equivalent required;
- LPN;
- Valid Driver's License.
