The Triage LPN at First Call Home Health (FCHH) provides essential support in managing patient care coordination, clinical orders, and communication within the home health setting. This position is responsible for triaging patient needs, managing physician orders and follow-ups, coordinating schedules, and ensuring the timely delivery of care services. The Triage LPN works closely with the Case Manager (CM), patients, caregivers, and the clinical team to facilitate effective and efficient care delivery. Additionally, the Triage LPN will perform PRN visits when necessary, particularly when field clinicians are unavailable, to ensure continuity of care.

This role is a support position, requiring the employee to adopt a supportive and collaborative approach to assisting clients, staff, physicians, and community members. As such, the In-Office Triage LPN must demonstrate professionalism, kindness, and a noncritical, non-judgmental attitude when interacting with all individuals.

4. PRN Visit Duties:

  • Perform PRN Visits: When field clinicians are unavailable, the In-Office Triage LPN will conduct PRN visits for patients as needed.
  • Follow Care Plans: The Triage LPN will follow the established care plan during PRN visits, ensuring all required treatments, assessments, and documentation are completed accurately and according to patient needs.
  • Document Visits: Document the PRN visit thoroughly in the patient's medical record, including all assessments, interventions, and outcomes from the visit.
  • Communication with Case Manager (CM): Notify the Case Manager (CM) of the PRN visit, detailing the reason for the visit and any findings or changes in the patient's condition that may affect the care plan or treatment.
  • Coordinate with Scheduling: Ensure that all PRN visits are scheduled appropriately and that the clinical team is aware of any updates to the patient's care plan or visit requirements.

8. Documentation and Communication:

  • Maintain accurate, up-to-date documentation for all orders, patient communications, and clinical coordination activities in the patient's EHR (electronic health record).
  • Ensure that all communication is documented in a clear and professional manner, adhering to HIPAA guidelines.

10. Other Duties as Assigned:

  • Participate in team meetings, case reviews, and quality improvement initiatives.
  • Perform other tasks as directed by the clinical management team to ensure the smooth operation of First Call Home Health (FCHH).

Qualifications:

Licensure & Certifications:

  • Active and valid LPN license in Oregon.
  • Current CPR certification (BLS or equivalent).

Experience:

  • Minimum of 1-2 years of clinical experience, preferably in home health or triage.
  • Experience managing orders, care plans, and documentation in an EHR system.
  • Experience with lab management, INR tracking, and follow-up care is preferred.

Skills and Abilities:

  • Strong clinical assessment, organizational, and critical thinking skills.
  • Ability to triage patient concerns effectively and make appropriate care decisions.
  • Excellent communication skills, both verbal and written, with the ability to coordinate across various teams and disciplines.
  • Pleasant & Courteous Communication: Ability to maintain a positive, friendly, and professional tone when interacting with patients, physicians, caregivers, and colleagues, ensuring that all communications are respectful and empathetic.
  • Supportive Approach: Must be able to take a supportive approach with patients, staff, physicians, and community members, demonstrating professionalism and kindness in all interactions and refraining from any form of criticism or judgment.
  • Knowledge of home health care regulations, practices, and documentation requirements.
  • Ability to work independently while managing multiple tasks and priorities.

Physical Requirements:

  • Ability to sit for extended periods.
  • Ability to communicate effectively via phone, email, and in-person with patients, families, and clinical staff.
  • Occasional lifting of office equipment or files (up to 20 lbs.).
  • Meet the physical requirements of field LPN staff.

Work Environment:

  • Primarily office-based with interaction with patients, clinical staff, and physicians.
  • Some occasional in-home visits or coordination may be required.

Job Type: Full-time

Pay: Up to $36.00 per hour
