We are looking for a Housing Specialist to work in our Housing, Transition and Tenancy Service.

The Housing Specialist is responsible to provide direct and indirect support to individuals as needed to achieve desired outcomes, obtain and maintain desired and permanent housing within the community, and identify appropriate resources.

Responsibilities include conduct initial screening for services, identify and locate budget and financial resources as supports or barriers to obtaining and accessing independent living; voucher programs, income-based apartments, and affordable market-based housing. The housing specialist will manage a caseload of individuals that need permanent housing in the following service areas: Cumberland, Dauphin, York, Perry and Franklin/Fulton Counties.

The Housing Specialist will develop a housing support crisis plan within 60 days from intake which includes prevention, for when housing is jeopardized, to assist with planning, locating and maintaining their home. Tracking and utilizing data on goals to measure progress in the quarterly progress note. Assisting with the process of searching for a home which is located within the desired community and ensuring the living environment is safe and ready for occupancy. Assisting and arranging for/support the details of the move. This does not include participating in the physical activities of moving items. Assistance with establishing and building relationships with providers within regulatory and professional guidelines.

Helping individuals connect to resources that supplement their budgetary restrictions (e.g. foodbanks, utility assistance, household furnishings…etc.)

Work collaboratively with all team members by using professional, strength-based language in person and in writing; Communicate objectively about needs.

Tenancy service includes providing early identification and intervention for behaviors that may jeopardize housing, such as late rental payments, lease violations, poor home conditions, and community relations.

· Assistance with communicating with landlord, developing relationships with neighbors/landlord to prevent eviction or adverse lease actions, and supporting the participant to understand lease terms and expectations.

· Assessment on the need for further education and training on the role, rights, and responsibilities of tenancy and provision of education if needed.

· Advocacy and linkage to legal resources to prevent eviction when housing is or may be jeopardized.

Time management; scheduling appointments, confirming and re-scheduling, completion of daily documentation.

Writing skills-required Quarterly progress noting, documentation of daily activities and email correspondence.

Proficient with Microsoft office suite, SharePoint, Microsoft Teams and Electronic Health Records application.

· Ability to solve problems as situations arise and/or seek support from The Cornerstone Agency of PA, Inc. team members.

· Displays environment appropriate presentation (dress, confidence, initiative, tactful), punctuality, and preparedness for appointments and meetings.

Requirements:

· Must be at least 18 years of age

· Education: A high school diploma required, associate or bachelor's Degree preferred

· Experience: At least one-year of experience in the field of human services with people who have an Intellectual Disability. A combination of internship, practicum, part-time employment, and related life experiences may be used to satisfy this requirement

· Clearances: Current Child Abuse Clearance, Criminal Background check, and FBI Clearances indicating no previous "founded" charges of child abuse or convictions that would compromise the person's ability to perform the job

· Must have and maintain reliable transportation, a valid PA driver's license, and automobile insurance

Job Type: Part-time

Pay: $ $27.00 per hour
