Job Summary
The Family Outreach Services Success Coach role ensures timely response and acceptance of referrals from New Mexico Statewide Central Intake (SCI). The Success Coach serves families who are reported and screened out for investigation for child abuse and neglect. The emphasis of case management is on assessing and addressing the family needs to better support any future prevention of child abuse and neglect.

Provide clients with supportive servicesCoordinate services between providers and clientsInterpret agency services to clients and communityProvide case management services and skill-building services to youth in the programMaintain individual case records on clients receiving servicesProvide specialized case management services to individual client needsEducate clients on available community resources and servicesCreate programs and services to benefit clients and the communityAttend community meetings relevant to program and clientsWhat you'll Need:
Bachelor's degree in social work or related human service fields.At least 1 year of internship or post graduation experience OR two years of combined experience working with communities on health or social service related matters, case manager experienceExcellent communication skills, both written and verbalExhibit skills in comprehending, interpreting, and completing delegated tasks efficientlyCompensation & Benefits:
