JOB SUMMARY: The Case Manager for our OASAS MRT program, you will play a major role in supporting single adults' families with clinical conditions in scatter-site supportive housing. As a Case Manager in this program, you are responsible for ensuring that all single adults and families receive support tailored to their unique circumstances, helping them navigate challenges, achieve stability, and other resources. The Case Manager will be involved in a range of supportive activities, including but not limited to conducting detailed intakes, ongoing monitoring of progress, and ensuring the continuous maintenance of care plans.
EDUCATION AND WORK EXPERIENCE:
- MINIMUM: HS Diploma or equivalent
- MINIMUM: Two (2) years of field experience providing housing and/or case management services to homeless populations, people with negative substance use, and/or people with psychiatric illness.
- Bachelor's Degree in Social Work, Human Services, or related field, plus 1 year of experience providing housing and/or case management services to homeless populations, people with negative substance use, and/or people with psychiatric illness strongly preferred.
- Ability to travel within Brooklyn by public transit.
- Up to 75% of traveling required.
DUTIES AND RESPONSIBILITIES:
Intake Assessment & Assessment
- Conducts intake assessments, quarterly re-assessments, and monthly (at minimum) home visits.
- Develops and implements initial, annual, and quarterly service plans for a caseload of 22–25 clients.
- Conducts bi-annual case conferences with collateral providers.
- Completes entry, annual, and exit HMIS assessments.
- Provides referrals for various services as needed.
Housing Support & Property Coordination
Coordinates move-in for new clients and relocation for existing clients when necessary.
- Completes initial, annual, and updated apartment inspections.
- Assists clients in establishing utility services and monitors monthly payments to prevent service disconnection.
- Resolves apartment repair and maintenance issues through coordination with BAC maintenance staff and advocacy with landlords or building management.
Group Facilitation & Client Engagement
- Facilitates and co-facilitates tenant meetings.
- Facilitates Independent Living Skills (ILS) groups.
Financial Monitoring
- Calculates, collects, and tracks monthly rent payments to ensure clients make appropriate payments.
Resource Management & Coordination
- Compiles and maintains a resource directory of landlords and service providers.
- Maintains positive working relationships with collateral contacts.
Referral & Resource Linkage
- Refer clients to appropriate services including medical, dental, mental health, substance use treatment, SSI, SNAP, legal assistance, education, employment, and training.
- Follow up on referral outcomes to ensure effectiveness and completion of services.
Community Engagement & Outreach
- Participate in community outreach efforts to engage clients and promote available services.
- Dedicate up to 65% or more of work hours to community engagement and fieldwork, including occasional home visits.
Collaboration & Relationship Building
- Establish and maintain effective partnerships with community organizations, healthcare providers, and stakeholders to enhance service delivery and resource access.
Other
- Other related duties as assigned based on program needs.
KNOWLEDGE, SKILLS, AND ABILITIES:
- Knowledge of issues related to negative substance use, recovery, and mental health.
- Familiarity with electronic medical records (EMR).
- Excellent verbal, written, and interpersonal skills.
- Excellent organizational and record-keeping skills, as well as the ability to manage multiple cases.
- Comfortable with digital tools and software for project management, data tracking, and communication, with a commitment to leveraging technology to improve program outcomes. Sound judgment, critical thinking, problem-solving skills, and ability to multitask.
- Knowledge of fair housing regulations and laws pertaining to housing assistance programs
- Strong communication and interpersonal skills to effectively work with diverse populations
- Ability to assess client needs and provide appropriate support and resources
- Experience in crisis management or addiction counseling is beneficial
- Excellent verbal and written communication skills, including the ability to convey complex information clearly and sensitively to diverse audiences.
- Proficient in collecting, analyzing, and interpreting data to inform program decisions and demonstrate impact.
