Position Summary
This is a full-time telework role within the US.
Monday-Friday: 3 days a week working 8:30AM to 5PM in respective time zone.
- 2 steady evening shifts per week: 1 will be 11:30-8PM and the other will be 12:30-9PM, to support business needs in Eastern Time Zone.
- Rotating Saturdays (partial day) are required, per business needs. Typically every 4-6 weeks.
- 1 late Friday until 9PM ET on a rotating basis. Typically every 4-6 weeks.
Required Qualifications
The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote and influence member's decisions related to achieving and maintaining optimal health status.
Assessment of Members through the use of clinical tools and information/data review, conducts comprehensive evaluation of member's needs and benefit plan eligibility for available integrated internal and external programs/services.
Utilizes assessment techniques to determine member's level of health literacy, technology capabilities, and/or readiness to change.
Enhancement of Medical Appropriateness & Quality of Care:
- Application and/or interpretation of applicable criteria and guidelines, health/wellness management plans, policies, procedures, regulatory standards while assessing benefits and/or member's needs to enable appropriate utilization of services and/or administration and integration with available internal/external programs.
- Using holistic approach consults with supervisors, Medical Directors and/or others to overcome barriers to meeting goals and objectives.
- Identifies and escalates quality of care issues through established channels.
- Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health.
- Provides coaching, information and support to empower the member to make ongoing independent medical and/or healthy lifestyle choices.
- Interprets and utilizes clinical guidelines/criteria to positively impact members health Provides up-to-date healthcare information to help facilitate the member¦s understanding of his/her health status.
- Helps member actively and knowledgably participate with their provider in healthcare decision-making.
Monitoring, Evaluation and Documentation of Care
- Develops and monitors established plans of care, in collaboration with the member and/or attending physician, to meet the member's goals.
- Utilizes internal policy and procedure in compliance with regulatory and accreditation guidelines.
Required Qualifications
- Active and unrestricted compact RN licensure in state of residence with ability to apply for and obtain additional licenses required per account needs
- 3+ years as a Registered Nurse performing chronic condition assessments and education in a clinic, homecare, or hospital setting treating Adults
- Must possess or be willing and able to obtain high speed internet access and be hardwired to internet
Preferred Qualifications
Previous health and wellness coaching experience (Face to face, group coaching and/or telephonic)
- Managed care experience
- Experience with Microsoft Word, Outlook, Excel, EPIC, and comfortable using various computer programs and troubleshooting basic technical issues remotely with and without assistance
Education
Associate or Bachelor's degree required in Nursing

Pay Range
The typical pay range for this role is:
$54, $116,760.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
