experienced as Patient Care Coordinators
and who
speaks FLUENT English and Spanish
for our Valley Stream office.
Patient Care Coordinators are responsible for matching patients with the right home health aides, providing basic case management services and are responsible for maintaining communication with patients, office personnel, paraprofessional staff, and contract agencies.

Patient Care Coordinator Benefits working for Summit:
Starting salary $24 - $25 per hour (depending on experience)

Patient Care Coordinator Position Qualifications:

  • Experienced patient care coordinator with a minimum of at least one year of experience in home care coordination.

  • Excellent telephone and communication skills
  • Excellent organizational skills
  • Ability to maintain accurate schedules, notes and records
