The Intake Coordinator is responsible for managing the initial intake process for families including conducting assessments, gathering necessary information and ensuring timely and accurate documentation. Intake Coordinators engage with stakeholders and community members to evaluate the appropriateness of referrals.

Responsibilities
The Intake Coordinator is responsible for managing the initial intake process for families including conducting assessments, gathering necessary information and ensuring timely and accurate documentation. Intake Coordinators engage with stakeholders and community members to evaluate the appropriateness of referrals.

  • Screen ACS and community referrals for appropriateness of the program to support with the program's utilization and sustain regular communication with key stakeholders to cultivate networking relationships.
  • Coordinate outreach to the referral sources and families to arrange joint home visits to assess for families' interest and readiness to engage in services.
  • Conduct comprehensive initial assessments and obtain necessary consent forms to ensure a smooth start to treatment and enable clinicians to make informed decisions on the optimal approach to servicing families.
  • Create, manage and process cases in internal and external electronic systems in an efficient and timely manner to ensure families timely access to services.
  • Provide support and guidance to families through the intake process to ensure a smooth and efficient experience that meets their needs and expectations.
  • Participate in weekly/bi-weekly coaching sessions to review referrals and case assignments, identify progress and barriers, and promote continuous professional development for enhanced performance and growth.
  • Participate in team meetings and trainings to stay informed about child welfare best practice, ensuring continued professional development and improved service delivery.
  • Engage in outreach community events to enhance awareness of the services offered by NY Foundling.

Qualifications

  • Bachelor's degree in Social work, Psychology, or Sociology.
  • A passion for serving children and families. Previous experience in child welfare, social services, or a related field is preferred.
  • Excellent writing, communication, interpersonal, problem-solving, time management and organizational skills.
  • Ability to work independently and as part of a team. Proven self-management abilities including meeting deadlines, prioritizing multiple tasks efficiently, timely completion of documentation and maintaining accurate and up-to-date case records.
  • Ability to work effectively with a diverse, multidisciplinary team as well as a diverse client population.
  • Ability to receive feedback to professionally grow and/or improve and be flexible with programmatic needs and changes.
  • Must be comfortable and able to travel and visit families in their homes.
  • An understanding and commitment to complying with the New York State Central Register of Abuse and Neglect protocol (requirement to report any suspected abuse, maltreatment or neglect of any child).

Education Required
Bachelor's Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution
