Position Summary

The Program Supervisor-Residential at REACH plays a crucial role in implementing the organization's vision, mission, strategic plan, and path and is a key member of the residential programs supervisory team. The Program Supervisor is regularly the first level of contact and representation of REACH for participants, their families, and direct service professionals. The Program Supervisor-Residential is tasked with fostering a culture of excellence, collaboration, and continuous improvement in programs that support independent living and supportive living for our participants. This role provides oversight and direction to a team of direct support professionals, supervises program participants, ensures program compliance, and monitors and maintains funding resources for participants. The primary objective is to ensure the effective delivery of services and maintain quality interactions with REACH participants, their families, staff, and the community.

Minimum Essential Duties and Responsibilities

Leadership

  • Ability to lead, direct, and inspire staff, utilizing a leadership style that fosters an open, engaging, growth-minded culture within your team and the organization.
  • Demonstrate healthy leadership skills within the team.
  • Attend regional center trainings and other professional development opportunities as directed by the organization.
  • Actively participate in and implement all REACH safety programs and provide immediate crisis intervention/emergency assistance to participants and/or staff members within the organization, ensuring prompt reporting of SIRs and work-related injuries.
  • Develop, cultivate, and nurture both internal and external relationships, including those with external agencies.

Support and Oversight of Direct Service Professionals

  • Oversee a team of direct service professionals, including scheduling, and conducting weekly staff support sessions and coaching.
  • Effectively schedule and utilize staff to maximize revenue.
  • Create and monitor action plans for addressing poor attendance of employees under direct supervision.
  • Assist staff with problem-solving for challenges they encounter in their support of participants and develop an appropriate action plan promptly.

  • Utilize all aspects of the performance management process, including developing annual staff performance evaluations that clearly outline performance goals, expectations, and strategies, and conducting regular check-ins and coaching sessions.

  • Conduct continuous staff training to ensure that team members are equipped with the necessary skills and knowledge to provide high-quality services to participants, ensuring the incorporation of new and relevant techniques and strategies around industry philosophies and advancements to promote ongoing growth and development.
  • Partner with Talent Acquisition to conduct structured interviews aimed at assessing candidate qualifications and skills that effectively evaluate candidates' alignment with the role's requirements and the team's needs and make hiring recommendations.
  • Ensure staff submit daily progress notes documenting participant activities and achievements, and that reporting is consistently up-to-date.
  • Manage timecards for team members, ensuring accurate tracking and reporting of hours worked.
  • Promptly request any missing or expired compliance documentation from staff members and participants.
  • Review and approve expense reimbursement requests per REACH guidelines.
  • Ensure consistent application of REACH policies and procedures.

Support and Oversight of Individual Participant Services

  • Manage participant caseloads, ensuring that each individual's needs are met comprehensively.
  • Support and guide the participant and the team of direct service professionals to acclimate to his/her new residence at the onset of Support Living Services.
  • Assist each participant in exploring the portfolio of details of the kind of home and neighborhood they want to live in (i.e., house, apartment, mobile home, upstairs, downstairs, garden, etc.).
  • Support participants to have opportunities to increase their abilities, confidence, and quality of life and support to maintain an adequate level of health and safety in their respective residences.
  • Assist participants in creating a financial budget for the residence.
  • Oversee and maintain the health of each participant's financial budget and financial affairs related to expenditures.
  • Develop and implement SMART goals in participant reports, ensuring goals are Specific, Measurable, Achievable, Relevant, and Time-bound, to track participant progress effectively and comprehensively.
  • Collaborate with participants, care providers, and staff to establish and monitor participant goals using person-centered language and practices.
  • Meet regularly with the Director of Programs to review caseloads, schedules, progress, and documentation.
  • Ensure that services are tailored to meet the participant's evolving needs and preferences allowing them to receive support without having to relocate from the home of their choice.
  • Assist all participants in scheduling and locating appropriate medical services as needed or as scheduled.
  • Report special incidents to relevant agencies and follow both internal and external reporting protocols for the organization.
  • Ensure participant services are person-centered and compliant with HCBS standards.
  • Support and guide the advancement of ongoing participant interdependence with but not limited to members of his/her Circle of Support, direct service professionals, families, neighborhoods, and communities.
  • Assist the participant and his/her family in interviewing potential direct service professionals for team placement.
  • In collaboration with the program participant and staff, develop participant schedules and conduct monthly schedule reviews and updates.
  • Regularly review participant authorization status or SLS contract to identify upcoming expirations and take proactive steps to renew authorizations promptly.
  • Update and review participant attendance records daily to identify any patterns or trends, such as frequent absences.

  • Create and monitor action plans for addressing poor participant attendance.

  • Monitor participant goals and make recommendations for adjustments.
  • Proactively schedule, coordinate, and attend Individualized Program Plans (IPP).
  • Regularly review participant authorization status to identify upcoming expirations and take proactive steps to renew authorizations promptly.
  • Regularly monitor participant files and compliance documentation to ensure all necessary records and certifications are up to date.
  • Engage regularly with participants, families, and staff to establish and maintain effective working relationships with open communication, sharing of upcoming events, and encourage participation and support for fundraising initiatives.

Other

  • Use time management techniques to ensure a proactive approach to your workload, efficient workflow, and timely completion of tasks.
  • Communicate proactively with the Director of Programs regarding issues arising that impact participants, families, and staff to inform and strategize moving forward and/or if any challenges may affect timelines.
  • Other duties as assigned.

Other Requirements

  • Must be at least 18 years of age.
  • Successfully complete health and fingerprint screenings.
  • Must have reliable transportation and show proof of auto insurance to attend in-person meetings, conduct supervisor field audits, and travel between worksites and company offices.

Work Location: In person

  • Bachelor's degree in a human services-related field and demonstrated ability to provide staff training, supervision, and planning; or, in lieu of a degree three years of successful supervisory experience in a human services program.
