Latino Community Services, Inc. (LCS) is a comprehensive non-profit organization dedicated to reducing the further spread of HIV/AIDS among the Latino community and other populations at risk and to improve the quality of life and health outcomes of individuals affected by HIV/AIDS.

JOB ANNOUNCEMENT:

Early Intervention Specialist (EIS)

Full-time (35 hours per week)

Purpose: The purpose of the Early Intervention Specialist is to perform outreach, identify and engage newly and previously identified HIV-positive individuals. Conduct tracking and re-engagement to clients and create meaningful linkages to HIV services, medical care and support services. The Early Intervention Specialist will focus on racial and ethnic minority groups that are disproportionally impacted by HIV/AIDS and other health disparities.

Essential Functions: Key activities of the Early Intervention Specialist include, but may not be limited to:

  • Outreach and engagement in alignment with LCS Outreach Strategy which includes (events & social network testing.
  • Development of individualized intervention, care and referral plan for each client including specific steps taken to meet the identified needs.
  • Group/individual intervention/prevention session/workshops.
  • Make all appropriate referrals and linkage both within LCS and to external providers for medical and other intervention and support services.
  • Monitor and evaluate continued appropriateness of care plan on a monthly basis.
  • Advocate as necessary to help client obtain needed services.
  • Maintain required documentation and updated charts with required assessments and data management duties per state guidelines.
  • Maintain confidentiality of all client-related information
  • HIV testing as requested
  • Other duties as assigned by supervisor

Education and Requirements:

  • High school diploma or equivalent.
  • Familiarity and/or knowledge of communities in Hartford, Tolland and Middlesex Counties.
  • Experience with HIV/AIDS testing & counseling, certification preferred or must be willing to obtain certification within six months of employment at LCS.
  • Knowledge of medical services and community resources network preferred.
  • Excellent interpersonal and communication skills.
  • Bilingual preferred.
  • Must be able to work evenings, weekends and in non-traditional hours.

Must have a valid CT driver's license and reliable personal transportation a must.

Job Type: Full-time

Pay: $ $21.00 per hour
