We are a dynamic home health agency committed to providing compassionate, high-quality nursing care in patients' homes. We serve a diverse population of clients with chronic and acute health needs, and you will be an integral part of our weekend care team.

Responsibilities

  • Provide home health nursing assessments, care planning, and implementation for patients in their homes.
  • Administer medications, coordinate with physicians, monitor vital signs, manage wounds, and educate patients and families
  • Document all nursing care according to agency policy and regulatory requirements.
  • Communicate effectively with the weekday clinical team to ensure continuity of care.
  • Provide patient education; make appropriate referrals based on identified patient needs that will maximize independence and patient/family participation
  • Collaborates with the physician and other members of the health care team to ensure care is safe, effective and coordinated.
  • Complete all OASIS time point assessments timely as per agency parameters.
  • Adhere to professionalism safety protocols, infection control, and patient confidentiality.

RN Requirements/Qualifications:

· Bachelor's Degree in Nursing preferred

· Minimum of one year of home care experience required.

· Current license from Board of Nursing in Massachusetts.

· Valid driver's license as well as reliable transportation.

  • Demonstrates responsibility for self-direction
  • Demonstrates ability to learn and utilize computer technology
  • Demonstrates effective communication skills

Job Type: Part-time

Pay: From $45.00 per hour
