Job Summary:

We are seeking a compassionate and dedicated Physical Therapist (PT) to join our Home Health Care team. The ideal candidate will provide high-quality, patient-centered care to individuals in the comfort of their own homes. As a Physical Therapist in a home health setting, you will play a crucial role in helping patients recover, regain mobility, and achieve their personal health goals. Your services will support patients in maintaining their independence, improving their quality of life, and promoting overall well-being. Candidates must be reliable, punctual, and possess strong communication skills, with the ability to submit documentation in a timely manner. Reliable transportation is essential for traveling to patient homes.

Key Responsibilities:

• Conduct comprehensive patient assessments, including physical, functional, and medical history evaluations, to develop personalized care plans.
• Design and implement therapeutic interventions tailored to the individual needs of each patient.
• Educate patients and their families about proper movement techniques, home exercises, and injury prevention strategies.
• Collaborate with other members of the healthcare team, including nurses, occupational therapists, and physicians, to ensure a coordinated approach to patient care.
• Monitor patient progress and adjust treatment plans as necessary to achieve the best possible outcomes.
• Provide clear documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with regulatory and company standards.
• Ensure patient safety and comfort during therapy sessions in the home environment.
• Act as an advocate for the patient, providing support, encouragement, and emotional reassurance during their rehabilitation process.
• Maintain knowledge of the latest physical therapy practices, techniques, and regulatory requirements.

Qualifications:

• Doctor of Physical Therapy (DPT) degree or equivalent from an accredited program.
• Valid state licensure as a Physical Therapist.
• CPR and First Aid certifications; BLS certification required.
• Professional Liability Insurance.
• Physical exam, TB test, and Chest X-Ray required.
• Valid California Driver's License.
• At least one (1) year of experience as a Physical Therapist.
• Prior experience in home health care or outpatient settings preferred.
• Experience working with elderly patients or individuals with chronic conditions is a plus.
• Strong communication and interpersonal skills to effectively interact with patients, families, and interdisciplinary teams.
• Ability to work independently and manage a flexible schedule.
• Knowledge of physical therapy equipment and modalities used in a home health setting.
• Strong problem-solving skills and the ability to assess both physical and emotional patient needs.
• Proficient in documentation, clinical software, and basic office technology.
• Reliable method of transportation.
• Eligible for Employment in the United States.

Physical Requirements:

• Ability to lift, move, or assist patients as required for therapeutic exercises and mobility training.
• Ability to drive to patient locations, which may require travel in various weather conditions.

Pay: $ $125.00 per hour
