Job Title: Home Health LPN

Reports to: RN Case Manager

Pay: Up to $34 an hour, DOE; Monthly bonus up to $500/month

We are looking to immediately hire 1 qualified Home Health Licensed Practical Nurses (LPN). This position provides skilled nursing interventions in the treatment of patients' illness, rehabilitative needs and preventative care. Utilizes a holistic approach in the provision of ongoing assessments, problem identification and specific therapeutic interventions, teaching and training activities. Responsible to apply professional knowledge and skills in accordance with accepted standards of clinical practice, to facilitate problem resolution, and achieving individualized patient/client goals and outcome.

Full time strongly preferred. We have multiple territories throughout our 60 mile coverage area in the Willamette Valley. We currently have a need for our Dallas area. Your assigned territory will be discussed based on your preferred location closest to your home. Each LPN sees approx. 5-7 patients a day with an ongoing caseload of approximately 25-30 patients.

Essential Duties & Responsibilities

  • Assess and document patient's conditions, treatments, interventions, and patient's response at assigned nursing visits and within the scope of care for a LPN.
  • Reports progress, diagnostic results and responses to treatment to health care team.
  • Follows plan of care as established by RN (case manager)
  • Documents all needed information in the medical record, following policy.
  • Participates in the care planning process. Integrates care- planning input from therapies and medical social service.
  • Utilized principles of safety and infection control, including universal precautions.
  • Transcribes and executes physician orders in accordance to LPN scope of practice.
  • Works with other team members to help establish goals, objectives, and discharge plan for each patient. Reviews the progress towards the goals and objectives and revises plan as needed.
  • Participates in health promotion for both patient and family.
  • Reports unusual or emergent patient or personal situations to manager or case manager.
  • Demonstrates knowledge and compliance with payer eligibility requirements.
  • Participates in quality improvement plan, quality control applications, and any other departmental research or studies as requested by the department manager.
  • Demonstrates commitment to the department's goals and to the organization's mission, vision, and values.
  • Adheres to nursing profession discipline code of ethics.
  • Fosters positive internal and external client relations
  • Demonstrates priority setting and time management capabilities.
  • Fosters effective work relationships through conflict resolution, constructive feedback skills, and problem-solving abilities.
  • May assist in training and orientation of new employees and students.
  • Maintains confidentiality of all patient related information.
  • Performs other duties as assigned in response to patient, office, or facility needs.

Minimum Qualifications & Experience

  • LPN or LVN license
  • Current Driver's License and proof of Auto Insurance
  • Current CPR card
  • 1 year of experience as an LPN preferred
  • Home care experience preferred
  • 90% of Skills Checklist
  • Must be able to meet Physical Demands of the position

Physical Demand

Physical effort refers to manual effort and may include: lifting, moving, pulling, pushing, carrying, manipulating, shoveling, etc. Because our services are rendered at the residence of our patients, it may demand some effort on the part of each employee to be physically able to climb stairs, be able to bend and lift, reach and stand. Employee must be able to lift a minimum of 50 pounds.
