JOB SUMMARY Under the supervision of the Substance Abuse Clinical Director, the Substance Abuse Intake Specialist provides high quality services to each client seeking services through Franklin Primary Health Center's, general outpatient substance abuse program. This person acts as the first point of contact for the Franklin Primary Substance Abuse Program. The intake specialist promotes programs within the community by providing education and information as necessary and recruits clients to maintain operation at acceptable capacity. The intake specialist coordinates and maintains relationships with partner agencies and legal representatives. The intake coordinator schedules appointments for official screening in order to determine applicants' eligibility to enter the FPHC SA outpatient treatment program, completes intake paperwork, and prepares client records for filing. The intake specialist ensures that client selection and admittance is in compliance with established program policies and procedures, orients new clients and their families to the program by explaining requirements, rules, and regulations, makes changes to the active and group rosters, and serves as a liaison to outside agencies, Probation Officers and the courts.

ESSENTIAL FUNCTIONS:

  1. Schedules interviews for applicants to meet with the Assessment Specialist for internal placement or external referral.

  2. Processes admitted applicants by orienting them to the program services, explaining program attendance requirements, rules and regulations, obtaining necessary signatures on formal agreements, initiating case records.

  3. Serves as liaison to outside agencies in order to refer clients to outside programs; confers with Probation/Parole Officers to arrange for on-site inspection and terms and condition of probation/parole when applicable; maintains regular contact with referral sources to/from other agencies and resources.

  4. Inputs client information into the computer throughout the clients' admission in the program; ensures the maintenance and security of all client files; files case records, records interview summaries, and verifies information.

  5. Provides clients upon admission with a summary of Franklin Primary Health Center's privacy practices and generates all applicable "Consent for release of information" forms, consent for treatment, emergency contact, client rights and responsibilities, and the grievance, complaints, and appeals process for outpatient treatment program clients.

  6. Answers inquiries regarding information on program services.

  7. Maintains a current waiting list and admits clients according to the current prioritization requirements of the Alabama Department of Mental Health contract; makes waiting list available to the designated office staff for reporting purposes; upon acceptance into the FPHC SA program, advises the new clients of the intake date.

  8. Assigns clients to therapist fairly, ensuring that all therapists keep an equal case load.

  9. Orients clients to the facility.

  10. Attends and participates in all staff, production, and clinical meetings or other scheduled meetings as assigned.

  11. Participates in staff trainings and ongoing supervision.

  12. Will provide services as delineated in the most recent version of the ADMH Substance Abuse Services Billing Manual as published and maintained by ADMH.

QUALIFICATIONS:

EDUCATION AND EXPERIENCE:

Two-year college or technical school degree in social work or a related field, (QSAP III preferred)

and

Two years progressively responsible experience performing clerical work as well as experience performing social services or public assistance work,

or

any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

A QSAP III shall consist of an individual who:

a. Has a Bachelor's Degree from a nationally or regionally accredited university of college in psychology, social work, community, rehabilitation, or pastoral counseling, family therapy, or other behavioral area that requires equivalent clinical course work and participates in ongoing supervision by a certified or licensed QSAP I for a minimum of one (1) hour individual per week until attainment of a substance abuse counselor certification credential from the Alabama Association of Addiction Counselors, National Association of Alcoholism and Drug Abuse Counselors, or Alabama Alcohol and Drug Abuse Association, or International Certification and Reciprocity Consortium/Alcohol and other Drug Abuse Inc..

KNOWLEDGE, SKILLS & ABILITIES: Knowledge of case management practices and services to evaluate patient care. Knowledge of organizational policies, regulations and procedures to administer assessment and treatment plans for patient care is required. Skills in establishing and maintaining effective working relationships with patients, medical staff, and the public are required. Skills are required in preparing and maintaining records, writing reports, and responding to correspondence. Abilities to maintain quality control standards. Abilities to interpret, adapt and apply program guidelines and procedures. Must possess the ability to communicate clearly and maintain confidentiality.

CERTIFICATION, LICENSES, REGISTRATIONS: Possession of a valid driver's license, access to a private vehicle for day-to-day job performance and auto liability insurance.
