Clinical Liaison for Long Term Care Facility

Position Summary
The Clinical Liaison serves as the primary connection between the facility, hospitals, physicians, and community referral sources. This role is responsible for evaluating potential residents for appropriate placement, coordinating admissions, and promoting the facility's services to healthcare providers and the community. The Clinical Liaison ensures a smooth transition for patients from acute care or home settings into the facility while maintaining compliance with all regulatory and organizational standards.
Key Responsibilities
Referral Management:
Receive and review all referrals from hospitals, physicians, and other sources.
- Conduct clinical assessments (onsite, via chart review, or telephonically) to determine the appropriateness of admission based on medical and psychosocial needs.
- Collaborate with physicians, discharge planners, and case managers to coordinate patient transitions.
Admission Coordination:
Work closely with the admissions team to ensure timely and accurate completion of admission paperwork.
- Communicate with families and patients about services, care plans, and facility expectations.
- Ensure all pre-admission screenings and authorizations (insurance, Medicare/Medicaid, managed care) are completed.
Marketing and Relationship Building:
Develop and maintain positive relationships with hospital case managers, physicians, and other referral sources.
- Conduct facility tours for prospective residents, families, and referral partners.
- Represent the facility at community events, health fairs, and networking meetings.
Clinical Collaboration:
Work closely with nursing and therapy teams to ensure seamless handoff and continuity of care.
- Provide clinical input to admissions and marketing staff regarding patient suitability and facility capabilities.
Compliance and Documentation:
Maintain accurate and up-to-date referral, assessment, and admission records.
- Ensure compliance with HIPAA, CMS, and state/federal regulations related to admissions and patient information.
Qualifications
- Education: healthcare background
Experience:
Minimum of 2 years of experience in long-term care, skilled nursing, rehabilitation, or hospital discharge planning.
- Prior experience in marketing, admissions, or liaison role strongly preferred.
Skills:
Strong clinical assessment and critical thinking skills.
- Excellent interpersonal and communication abilities.
- Knowledge of Medicare, Medicaid, and insurance processes.
- Ability to multitask in a fast-paced environment and work independently.
