Home Health PT Physical Therapist – Empath Home Health – Fort Myers

Empath Home Health is growing We're expanding our home health services in Fort Myers and looking for a passionate Home Health PT Physical Therapist to join our team.

This is an exciting opportunity to serve patients in their homes with the flexibility to set your own schedule and grow with us as we continue to build our presence in the community. In this full-time role, you'll deliver individualized care, build trusted patient relationships, and help people manage their health with dignity and independence.

Locations: home health visits in Lee county (Fort Myers, North Fort Myers, Sanibel, Lehigh Acres, Bonita Springs, and surrounding areas)

Schedule: Full time; M-F 40 Hours. Requires weekend/holiday on-call rotations

With more than 60 years of expertise, Empath's Home Health service line provides skilled nursing, therapy, and personal care services that help people recover and maintain independence at home. Part of Empath Health's not-for-profit network, the program serves patients across 16 Florida counties and is accredited by the Joint Commission and the Accreditation Commission for Health Care.

What you'll Do

• Provide physical therapy services to patient as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations.
• Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient's ability to perform activities of daily living.
• Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility.
• Reports to the physician and designated agency representative patient's reaction to treatment and any changes in the patient's condition.

What You'll Need

• Active Florida PT license
• Minimum of 1 year of direct patient care experience; prior home health PT experience preferred
• Home health care experience strongly preferred.
• Prefers experience using Homecare Homebase (HCHB) EMR
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
