Job Overview

We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will possess a strong background in patient care and rehabilitation, with a focus on providing high-quality therapy services across various settings, including inpatient, outpatient, and home health. This role requires expertise in developing individualized care plans and implementing effective treatment strategies to enhance patient recovery and overall well-being.

Responsibilities

  • Conduct comprehensive evaluations of patients to assess their physical abilities and limitations.
  • Develop and implement personalized physiotherapy treatment plans based on individual patient needs.
  • Provide therapeutic interventions, including manual therapy techniques such as trigger point therapy, and exercise prescription for various conditions.
  • Collaborate with interdisciplinary teams to ensure coordinated care for patients in acute care, outpatient, and home health settings.
  • Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
  • Educate patients and their families about treatment options, exercises, and self-care strategies to promote independence in daily activities.
  • Maintain accurate documentation of patient assessments, treatments, and progress using EMR systems.
  • Stay current with best practices in physical therapy through continuous education and professional development.

Experience

  • A degree in Physical Therapy from an accredited program is required.
  • Proven experience in various settings such as inpatient hospitals, outpatient clinics, pediatrics, geriatrics, orthopedics, or home & community care is preferred.
  • Strong knowledge of anatomy and physiology as well as medical terminology is essential for effective patient assessment and treatment planning.
  • Familiarity with discharge planning processes and the ability to develop comprehensive care plans tailored to individual patient needs.
