As a Home Health Physical Therapy Assistant (PTA), you will work under the supervision and direction of a licensed Physical Therapist (PT) to provide skilled rehabilitative care that enhances mobility, strength, and independence. You will deliver individualized interventions to patients recovering from illness, injury, or surgery, and help them achieve their highest functional potential in the comfort of their home. Essential Duties and Responsibilities:

• Implement individualized Plans of Care (POC) developed by the supervising PT in accordance with physician orders and home health regulations.
• Deliver therapeutic exercises, gait and transfer training, balance activities, and functional mobility interventions to promote safety and independence.
• Educate patients and caregivers on exercise routines, fall prevention strategies, body mechanics, and home safety modifications.
• Observe and document patient progress, changes in condition, and response to treatment; communicate findings to the supervising PT and interdisciplinary team promptly.
• Accurately and timely complete all required documentation and visit notes within the Electronic Medical Record (EMR) system.
• Collaborate with nurses, occupational therapists, and other care team members to ensure continuity and consistency in patient care.
• Participate in case conferences and interdisciplinary meetings as required to support coordinated care and patient progress.
• Maintain compliance with agency policies, Medicare Conditions of Participation (CoPs), and all regulatory and ethical standards.
• Assist in reassessments, goal adjustments, and discharge planning under the PT's supervision.
• Uphold the agency's mission by delivering compassionate, evidence-based care that prioritizes patient dignity, safety, and well-being.

Qualifications and Experience:

• Education: Graduate of an accredited Physical Therapist Assistant (PTA) program.
• Licensure: Current, active Physical Therapist Assistant license in the state of practice.
• Experience:
• Minimum of one year of clinical experience preferred; home health experience highly desirable.
• Proficiency in orthopedic, neurological, and post-surgical rehabilitation techniques.
• Familiarity with OASIS documentation and home health compliance standards preferred.
• Demonstrated ability to work independently while maintaining communication with supervising PT.
• Skills:
• Strong knowledge of anatomy, physiology, kinesiology, and therapeutic modalities.
• Excellent communication, observation, and critical-thinking skills.
• Reliable, organized, and committed to providing high-quality, patient-centered care.

Physical Demands:

• Must be able to safely lift and transfer patients up to 50 lbs.
• Requires frequent standing, walking, bending, and driving between patient homes.

Work Environment:

• Home-based settings including private residences, assisted living facilities, and community environments.
• Flexible scheduling available; travel required within assigned service area.

Pay: $ $75.00 per hour
