Conduct initial client assessments; develop case plans based on each client needs; provide direct support services to qualified applicants; coordinate with other agencies to address additional services, if needed. Document case details and enter each into the data management system and refer clients to addition resources if needed. Exercise excellent customer service techniques. Strong communication and interpersonal skills to build rapport with clients from diverse backgrounds; ability to conduct thorough assessments and develop comprehensive case plans; knowledge of social service programs; excellent documentation and reporting skills. Ability to work independently and as part of a team; commitment to ethical practices and client confidentiality; well versed in Microsoft Office; conduct initial client assessments, develop case plans based on client's needs. Document case details; enter in Data Management System. A bachelor's degree in pursuit or earned, or 2-year experience in social services or related field.
