Position Summary:

We are seeking a compassionate and highly organized Healthcare Coordinator who is either a Certified Medical Assistant (CMA) or a Licensed Practical Nurse (LPN) to join our dedicated team at our residential mental health treatment facility. The ideal candidate will play a key role in supporting our clinical operations, ensuring safe and effective medication management, and contributing to the overall wellbeing of our clients.

Pay: Between $ $26.00 an hour 

Responsibilities:

  • Support facilitators with milieu related meetings including enforcement of milieu rules and structure and supporting clients in adhering to the program expectations and responsibilities in treatment.  
  • Administer injections (e.g., psychotropic medications, vaccines)  
  • Place and read PPD tests 
  • Assist clients with self-administration of medication and document accordingly. 
  • Provide clients with transportation and support, as needed, for medical appointments, outside recovery meetings, grocery shopping trips, recreational outings and other appointments, as indicated.  
  • Oversee clients' self-administration of medication and report any concerns to the team.  
  • Participate in training and meet licensure expectations related to training.  
  • Conduct UA testing as needed.  
  • Fully understand and maintain policies regarding professional ethics, including appropriate boundaries and patient confidentiality; monitor documents according to company and HIPAA policies.  
  • Ensures clients are supported with coordinating nursing, medical, health-related or ancillary services, as indicated.  
  • In collaboration with the Clinical Director and Registered Nurse, support the clinical residential medication management program, ensuring accurate count of all medications and documentation in the EMAR. 
  • Coordinate new clients nursing and psychiatry appointments, and off-site medical & dental appointments as indicated.  
  • Communicates with off-site providers/PCP to obtain proper documentation for all off-site or non-Anew related healthcare appointments.    
  • Coordinate with Anew's Registered Nurse to support initial and in-service training for CSS regarding medication management policy and procedures. 
  • Develop and send-out weekly psychiatry schedule & add weekly appointments to Best Notes prior to the psychiatrist appointment with the client. 
  • Coordinate with Anews' psychiatrist, as indicated, following clients' weekly appointments and in accordance with any client needs that arise. 
  • Support the oversight of the health care management services for clients at the RBHF and Anew Outpatient Campus. This includes development and oversight of policies related to medication management, physical health care coordination, and other medical needs as required. 
  • Provide medication management services including intake into the electronic health record.  
  • Complete timely shift reports that convey clear and concise statements summarizing client's feelings, presentation and response in clinically significant encounters.  
  • Admission-related healthcare management needs, including tracking LIC required health assessments and needs.   
  • Model appropriate interpersonal relationships, emotional regulation, and boundaries.  
  • Attend and participate in assigned and required training, supervision, and weekly meetings.  
  • Performs other related duties as assigned.  

Qualifications 

  • Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) preferred and in good standing
  • Minimum 1 year of experience in a behavioral health or psychiatric care setting (preferred)
  • Preferred candidates should have sound understanding of mental illness, psychotropic medications, co-occurring disorders, and milieu therapy.  
  • Excellent organizational and time-management skills
  • Ability to work both independently and as part of a multidisciplinary team
  • CPR certification (or willing to obtain upon hire)
  • Proficient with electronic medical records (EMR
  • Have a valid driver's license in good standing and auto insurance.  
  • Successfully complete drug screen and comprehensive background check.
