Job Overview

The Transition Coordinator plays a vital role in facilitating the smooth transition of individuals, particularly those with disabilities, into new programs or services. This position requires a compassionate and organized professional who is adept at managing relationships, overseeing program development, and ensuring that all administrative tasks are handled efficiently. The ideal candidate will possess a strong background in social work and behavioral health, along with experience in supervising teams and managing program budgets.

Responsibilities

  • Coordinate the transition process for individuals moving into new programs or services.
  • Develop and implement effective transition plans tailored to individual needs.
  • Supervise staff involved in the transition process, providing guidance and support as needed.
  • Manage program development efforts to ensure alignment with best practices and regulatory requirements.
  • Oversee budgeting processes related to transition programs, ensuring financial accountability.
  • Maintain strong relationships with stakeholders, including families, service providers, and community organizations.
  • Conduct assessments to identify the needs of individuals transitioning into new services.
  • Facilitate training sessions for staff on best practices in supporting individuals with developmental disabilities.
  • Ensure compliance with all relevant policies and regulations throughout the transition process.

Requirements

  • Bachelor's degree in Social Work, Psychology, or a related field
  • Proven experience in supervising teams and managing programs within the behavioral health or social services sectors.
  • Strong skills in program development and budgeting.
  • Excellent relationship management abilities to effectively engage with individuals, families, and community partners.
  • Knowledge of best practices for working with people with developmental disabilities.
  • Exceptional organizational skills and attention to detail.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Strong communication skills, both verbal and written.
