Provides direct support and evaluations of adults with developmental disabilities in a day habilitation setting. Serves as a member of an interdisciplinary team in the coordination of therapeutic services. This position has limited case management responsibilities.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:
- Accurately record all data for established goals, object and behavior plans of members in assigned group.
- Write progress notes for assigned members. Assist with all development of behavioral plans or other intervention strategies. Chart data collected from programs.
- Ensure member training programs are implemented according to their DHSP
- Establish oneself as the group coach/leader, assisting new staff in learning specific individual and/or group tasks and activities.
- Attend mandated Safety Care training. Responsible for implementing physical restraints on the individuals we support to assist with behavioral management. This may include, but not limited to, hitting, biting, kicking, and spitting.
- Develop curriculum, activities and modules to enhance the quality of services provided to members.
- Prepare reports in accordance with state and agency requirements.
- Work in cooperation with Paraprofessionals to develop member skills in the areas of: selfcare, self-preservation, expressive communication, self-direction, and activities of daily living.
- Serve as a liaison between RTR and the community representing a positive and proactive attitude at all times.
- Cooperate and interact with co-workers, clients, volunteers, and funders in a collaborative, courteous, empathic, and professional manner. Demonstrate willingness and ability to be part of team effort.
- Serve as an appropriate role model for individuals under supervision establishing behavioral limits in a positive consistent manner and in accordance with behavioral plans.
- Promptly inform Case Manager when a member is not working toward established goals.
- Attend and participate in all meetings concerning assigned individuals.
- Adhere to all program routines and schedules.
- Provide transportation to activities as needed.
- Assist and support individuals as needed with physical direct care/dietary needs, may include: physical or mechanical transfer of individuals from wheelchairs to a van, chair, toilet, etc. May also include any nursing activity that is deemed to be delegable, including, but not limited, to the administration of thickening agents, and assistance with special diets.
- Engage in remote/virtual services with program members following a preplanned curriculum. This includes the ability to use a computer and the ability to engage several individuals for an extended period of time, typically one hour or less. You may be asked to develop new curriculums to meet the needs of the individuals under your care.
- Expected to follow current OPTS/Day Hab guidelines based on DDS/MassHealth regulations as they relate to the COVID-19 virus. This includes daily health screening and wearing appropriate PPE as defined in the guidelines.
- Participate in the Positive Behavior Support initiative and complete the Quality Behavior Curriculum.
- Implement all behavioral plan interventions as assigned; these behavioral interventions may include physical restraint.
- Adhere to the schedules and needs of RTR and the schedules and needs of individuals under supervision.
- Adhere to agency safety standards.
- Perform additional duties, or substitute for others as required.
- Adhere to all RTR policies and procedures.
- Develop and provide all services in accordance with RTR's philosophies including staff and Board beliefs.
- High school diploma or GED
- Valid driver's license.
- Basic knowledge of Microsoft Word and the internet.
- Successful completion of all state and agency mandated trainings.
- Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies.
- Must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.
- Tuberculosis screening within the previous 12 months.
Must be fluent in English and have the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community.
