Golden Years Home Health Inc. is currently seeking a self-motivated Physical Therapist to deliver premier excellence of care and is enthusiastic about working with adults of all ages.

Our Licensed Physical Therapists (PT) work in collaboration with the doctor, patient and the patient support system to provide best in class care for return to health and activity. As a Physical Therapist, you are responsible for the evaluation of mobility, strength and level of function. With this information you will establish goals to improve balance, walking, strength, as well as patient education. In the comfort of the patients' home, the Physical Therapist assesses the home environment to ensure safe mobility is achieved. With your knowledge of best in class techniques, you will recommend equipment to enhance therapy, as well educate the patient on proper use.

Essential Functions:

  • Completes all assessments/evaluations, including OASIS if necessary, at specific time-points as defined by regulation and agency policy, accurately and timely.
  • Develops a plan of care in collaboration with the patient, family/caregiver and physician including frequency and duration of therapy visits, specific treatments, appropriate goals, other disciplines needed and discharge plan.
  • Performs and documents physical therapy interventions including patient education according to the Plan of Care and adapts these actions as patient needs change, ensuring optimal outcomes.
  • Communicates changes in the patient status, patient issues and concerns and changes in orders to the physician and the Clinical/Rehab Manager.
  • Designs and instructs a patient specific home exercise programs to patients, families and caregivers recommending and providing assistive equipment, as needed.

Rates are per visit basis. For our Therapists, that is $80- $135 per visit depending on type of visit Once you sign up to be a part of our discipline pool, you will be working hand in hand with our intake unit. The staff will be presenting you cases (depending on the demand), and it is entirely up to you to whether take or decline. Our company is very flexible, you control your own time and you may visit the patient depending on both your availability.

You must be willing to accept SOC, ROC and Recert Oasis. This position is not limited to evaluations only.
