Flagstar Rehab SLPs work with a diverse and multi-disciplinary team to perform evaluations, treatments, and patient/family education to improve the speech and swallow capabilities of their patients and improve ability to function in their daily lives. *

Hourly Pay Rate: $42-$48 Hourly (W-2), $55-$61 Hourly (1099)
Speech Language Pathologist Job Duties:

  • Provide efficient speech or swallow evaluations and/or screens of patients
  • Develop plan of care, downgrade/upgrade as appropriate, manage meal texture
  • Provide quality therapy services to individuals in an SNF setting, who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
  • Monitor and document patient progress with the use of innovative EMR software.

Speech Language Pathologist Requirements:

  • Not accepting Clinical Fellows (CFY) at this time.
  • Current NYS SLP License and Registration
  • Clinical Competence Certification (CCC)
  • CPR Certificate
  • EMR Competence
  • State Required Medical Documentations for work in a Skilled Nursing Facility (Physical, TB Test, MMRV Vaccine Record or Titers, Covid Vaccine Documentation)
  • Valid Proof of Identity and Work Authorization
  • Malpractice/Liability Insurance (if applicable)
