We are expanding and seeking compassionate, motivated Nurse Practitioners to join our SNF/Post-Acute team.
Position Summary
As an SNF Nurse Practitioner, you will manage an assigned patient panel within partnering facilities, provide comprehensive evaluations, monitor acute and chronic conditions, and collaborate closely with the interdisciplinary team to ensure continuity of care.
This role is ideal for providers who thrive in a flexible, independent, and collaborative clinical environment.
Key Responsibilities
- Conduct comprehensive assessments, including medical histories, physical exams, and diagnostic evaluations
- Manage acute and chronic medical conditions for patients in SNF/Post-Acute settings
- Develop and implement evidence-based treatment plans
- Perform follow-up visits, medication management, and ongoing clinical monitoring
- Collaborate with facility staff, physicians, therapists, and nursing leadership
- Document encounters thoroughly in accordance with regulatory and payer requirements
- Participate in care plan meetings and quality-improvement initiatives
- Communicate effectively with patients' families regarding health status and care plans
Provide on-call coverage as scheduled (if applicable)
Qualifications
- Active Texas Nurse Practitioner license (FNP, AGNP, or ACNP preferred)
- Board certified and able to practice independently in Texas
- Experience in SNF, Post-Acute Care, or Geriatrics preferred (but not required — training available)
- Strong clinical judgment and ability to work autonomously
- Compassionate, patient-centered approach
- Excellent communication and documentation skills
Job Type: Full-time
Pay: $100, $102,719.41 per year
Work Location: In person
