Job Title: Home Health and Hospice Clinical Liaison
Reports To: Vice President Clinical Business Development
Department: Home Health and Hospice Services
FLSA Status: Exempt / Non-Exempt
Job Summary:
The Home Health Clinical Liaison serves as a bridge between healthcare providers, patients, and home health and hospice services. This role is responsible for assessing patient eligibility for home health care and hospice care, coordinating referrals, and ensuring a smooth transition from hospital or other care settings to home health and hospice services. The Clinical Liaison fosters strong relationships with healthcare professionals and facilities to drive patient referrals and enhance the organization's presence in the community.
Key Responsibilities:
Patient Assessment & Coordination:
- Evaluate patients for home health eligibility based on clinical criteria and regulatory guidelines.
- Collaborate with hospital discharge planners, physicians, and case managers to facilitate seamless transitions to home health care.
- Educate patients and families on home health services, care plans, and expectations.
- Obtain necessary documentation, consents, and physician orders for home health services.
Referral Management & Business Development:
- Develop and maintain strong relationships with hospitals, skilled nursing facilities, physicians, and other healthcare providers to drive referrals.
- Consistently achieve established goals.
- Create and develop opportunities for growth with hospitals.
- Communicate effectively with referral sources to ensure a thorough understanding of home health benefits and services.
- Track and analyze referral trends and outcomes to optimize patient acquisition strategies.
- Identify and set up C-Suite meetings for our C-Suite to collaborate and partner with hospitals.
- Identify and work with leadership to establish preferred partnerships, ACO relationships, narrow network opportunities, electronic referral opportunities and other ways to integrate with hospitals and health systems.
Clinical Support & Compliance:
- Ensure all referrals meet home health regulatory and compliance requirements.
- Work closely with the clinical team to ensure timely initiation of home health services.
- Maintain accurate documentation in accordance with state, federal, and organizational policies.
- Provide training and education to referral sources on home health regulations, eligibility, and best practices.
Community Outreach & Education:
- Represent the organization at community events, professional meetings, and networking opportunities.
- Conduct in-service presentations for hospital staff, physicians, and other referral partners in hospitals.
- Stay informed of industry trends, regulatory changes, and best practices to support growth initiatives.
Qualifications:
Education & Experience:
- Registered Nurse (RN), Licensed Practical Nurse (LPN), Physical Therapist (PT), Occupational Therapist (OT), or other qualified clinical background preferred.
- Minimum of 2 years of experience in a healthcare setting, preferably in home health, hospice, or discharge planning.
- Experience in sales, marketing, or business development within the healthcare industry is a plus.
Skills & Competencies:
- Strong knowledge of home health regulations, Medicare/Medicaid guidelines, and reimbursement processes.
- Excellent communication, interpersonal, and relationship-building skills.
- Ability to work independently while collaborating effectively with interdisciplinary teams.
- Strong problem-solving skills and attention to detail.
- Proficiency in electronic health records (EHR) and Microsoft Office Suite.
Work Environment & Physical Requirements:
- Ability to travel to hospitals, skilled nursing facilities, physician offices, and other referral sources.
- Occasional evening or weekend work may be required for community events or patient needs.
- Must be able to sit, stand, and move frequently in various healthcare settings.
