Position SummaryWe are seeking an Intake and Facility Relations Coordinator to join our growing team. This individual will be responsible for building and maintaining strong relationships with hospitals, rehab centers, and assisted living facilities while managing new client intakes from referral to admission.
The ideal candidate has experience in healthcare, home care, or facility relations and enjoys connecting with people while ensuring all admissions are handled efficiently and compliantly.
Key Responsibilities- Develop and maintain relationships with discharge planners, social workers, and case managers at hospitals, rehab centers, and facilities.
- Serve as the first point of contact for incoming referrals and new client inquiries.
- Gather client information and coordinate admission paperwork.
- Collaborate with HR, scheduling, and nursing teams to ensure seamless client onboarding.
- Track referral sources and maintain an updated facility contact database.
- Provide weekly reports on referrals, admissions, and outreach activities.
- Represent the agency professionally during facility visits and community events.
- Previous experience in home care, healthcare intake, or facility outreach preferred.
- Strong communication, organization, and relationship-building skills.
- Should know how to use DocuSign for client and staff paperwork.
- Valid driver's license and reliable transportation required.
