Job Overview

We are seeking compassionate and dedicated Home Health Aides (HHA) and Certified Nursing Assistants (CNA) to provide high-quality care on a PRN basis. This role involves supporting patients in their homes or assisted living facilities, assisting with daily activities, and ensuring their comfort and safety. Ideal candidates will have experience working with diverse populations, including seniors, individuals with developmental disabilities, dementia, or those requiring hospice care. The position offers flexible scheduling to accommodate both patients' needs and caregivers' availability.

Responsibilities

  • Monitor and observe patient condition, reporting any changes or concerns promptly
  • Administer medication and perform basic medical tasks such as vital signs measurement and catheter care regulations
  • Support patients with memory care needs, Alzheimer's or dementia care, and hospice-related services
  • Assist with meal preparation, feeding, laundry, light cleaning, and maintaining a safe environment
  • Provide emotional support and companionship to enhance patient well-being
  • Follow care plans developed by healthcare professionals and communicate effectively with team members

Requirements

  • Valid HHA or CNA certification required; CPR certification preferred
  • Experience in assisted living, nursing homes, or home health preferred; experience with long-term care or working with people with disabilities is a plus
  • Knowledge of medical terminology and documentation procedures
  • Ability to work independently while adhering to care plans and safety protocols
  • Strong communication skills and compassionate attitude toward patients of all ages and backgrounds
  • Experience working with individuals with dementia, Alzheimer's, developmental disabilities, or hospice needs is highly desirable

This position offers flexible hours on a PRN basis to meet the needs of our clients while providing meaningful support to those in need of home or community-based care.
