Position Summary:

The Nurse Practitioner (NP) provides comprehensive, patient-centered primary care services, including assessment, diagnosis, treatment, and preventive health management. Working independently and collaboratively with physicians and other healthcare professionals, the NP ensures high-quality care focused on improving patient outcomes and overall wellness.

Essential Duties:

  • Conduct comprehensive physical exams and health assessments.
  • Diagnose and manage acute and chronic conditions.
  • Prescribe medications, therapies, and diagnostic tests as appropriate.
  • Develop individualized care and treatment plans for patients.
  • Provide patient education on health promotion, disease prevention, and lifestyle management.
  • Collaborate with interdisciplinary care teams including physicians, nurses, behavioral health specialists, and social workers.
  • Maintain accurate medical documentation and ensure compliance with healthcare regulations.
  • Participate in continuing education to maintain certification and stay updated on medical advancements.
  • Support quality improvement initiatives within the clinic or care setting.

Education and Experience Requirements:

  • Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program.
  • Active and unrestricted Nurse Practitioner license in the State of Indiana.
  • National certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology NP.
  • Minimum 2 years of experience in a primary care or family practice setting preferred.
  • Strong clinical judgment, communication, and patient care skills.
  • Experience working in diverse, community-based care environments is a plus.

Job Type: Full-time

Pay: $81, $120,000.00 per year
