In-Home Care Agency

Position Overview

We are seeking an organized, compassionate, and confident Client Care Coordinator to oversee client care services and support our caregiving team. This role serves as a key link between clients, caregivers, and office leadership, ensuring high-quality care, compliance with care plans, and strong caregiver performance.

The Client Care Coordinator plays a hands-on leadership role by assisting with caregiver training, performance management, and client care oversight while helping maintain excellent communication and service standards across the agency.

Key Responsibilities

Client Care Oversight

  • Oversee client care services to ensure care plans are followed accurately
  • Conduct client check-ins, home visits, and follow-ups as needed
  • Monitor quality of care and address concerns promptly
  • Communicate changes in client condition to appropriate team members
  • Assist with care plan updates and service adjustments

Caregiver Training & Support

  • Assist with onboarding and training of new caregivers
  • Provide ongoing coaching, mentoring, and performance feedback
  • Serve as a resource for caregivers with questions regarding care tasks or client needs
  • Support caregivers in the field to promote confidence and consistency

Performance Management & Discipline

  • Assist with caregiver disciplinary processes, including documentation and coaching
  • Support corrective action plans in collaboration with leadership
  • Address attendance, performance, and conduct issues professionally and timely
  • Participate in investigations related to care concerns or complaints

Administrative & Office Support

  • Maintain accurate documentation related to client care and caregiver performance
  • Collaborate with scheduling, care advocates, and leadership teams
  • Support compliance with agency policies, state regulations, and care standards
  • Assist with audits, reports, and quality improvement initiatives as requested

Qualifications

  • Prior experience in in-home care, healthcare coordination, or caregiving leadership
  • Strong understanding of caregiving tasks and client care needs
  • Excellent communication, organization, and documentation skills
  • Ability to manage sensitive situations professionally and compassionately
  • Strong problem-solving and decision-making abilities
  • Comfortable working with diverse client populations and caregivers
  • Valid driver's license and reliable transportation
  • Ability to pass background check and meet agency/state requirements

Preferred Experience

  • Experience in caregiver supervision, training, or care coordination
  • Familiarity with care plans and service documentation
  • Leadership or management experience in home care or healthcare

Pay: $ $22.00 per hour
