Summary:
The Client Care Coordinator works closely with each of the Clients on an individual basis, providing assistance in the areas of, medication observation and medical appointment scheduling, as well as overall client health. The Client Care Coordinator ensures that all information obtained during the course of client's stay is organized and facilitated appropriately. The Client Care Coordinator adheres to all state and federal regulations and accreditation agency standards applicable to the individual licensure treatment programs. Supervises other CCC's
Essential Responsibilities and Duties
Observation, documentation, audit functions, and record keeping of all observed/inventoried/destroyed/prescribed medicationsA licensed and/or certified facility staff member, RN/LVN, completes the assessment (DHCS Health Questionnaire) in the clients' EMR at Intake. The assessment will be done prior to the client's receiving Incidental Medical Services.Observation, documentation, and record keeping of client health status.Works closely with Operations Manager/Clinical Director/Therapist/ and Case Managers on client health statuses.Manages Client Care data in the EMR such as orders,Pick up medications/ other medical supplies/ interact with client's Pharmacy/PharmacistSet all medical appointments for clients including medical, dental, and psychiatric as needed.Work with Operations on Medication refillsCommunicate with local hospitals, doctor's offices, urgent cares, etc.Communicate with families when release is valid for updates as needed.Responsible for all medical records requests from outside providers while in our care.Understanding and ability to score COWS and CIWA for withdrawal of symptoms during detox.Utilize the 6 Rights before clients self-administer medication.Ensure that an adequate supply of medications, supplies, and equipment is on hand to meet the needs of the clients.Ensure that narcotic records are accurate for your shift. Narcotic count must be done between all shifts and after each self-administration of said medication.Dispose of drugs and narcotics as required, and in accordance with federal and state guidelines and company established procedures.Make daily rounds of your shift to ensure clients are doing well.Completion of all client process duties and/or documentation within timelines:Responds to all social service needs, requests by physicians, legal issues, and family needs.Crisis InterventionObserve, evaluate and report on behavior, attitudes general emotional stability clients and their family members to help assist in treatment planning/discharge planning by assessing social needs of individual clients including referrals to outside providers after transition to outpatient therapy.Will be provided performance evaluations to measure performance areas. Inability to meet performance expectations will result in specific, short term performance enhancement action plan with a timeline to improve. Below average rating in employee performance will result in progressive disciplinary action up to and including termination of employment.*The job duties listed above should not be construed as an exhaustive list of all job duties required. Qualifications and Skills
Must possess, as a minimum, an LVN license; must possess a current, unencumbered, active license to practice in California and/or Medical Assistant certificate under supervision of LVN.Must be able to read, write, speak, and understand the English language.Must possess the ability to make independent decisions when circumstances warrant.Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.Must be able to communicate/relate information concerning client's status with family members.Must be able to cope with the mental and emotional stress of the position.Must have treatment experience with strong professional references attesting to high degree of clinical sophistication.Current certification in First Aid/CPRAdvanced knowledge of HIPAA and CFR 42 RequirementsCurrent TB test clearance
