The Role
The
Sr. Case Manager
will join the Economic Prosperity Services team, reporting directly to the Associate VP of Economic Prosperity Services. As a dedicated and skilled professional, the Sr. Case Manager will serve as the lead case manager and supervisor of the case manager's team serving primarily our housing residents and coaching clients.
In collaboration with various agency partners, the Sr. Case Manager will support individualized household needs and facilitate tailored resident Action Plans.
This role is ideal for an experienced social service professional who is passionate about making a positive impact in the community by supporting staff and the families that live in JPNDC's housing portfolio.
Duties
- Primary liaison for residents in the JPNDC housing portfolio who have or may have expressed a need for social services.
- Primary liaison for property managers, asset management team and community agencies.
- Attend regular meetings with the Property Management Company regarding rental arrearages and eviction prevention procedures.
- Supervise and support a team of case managers.
- Responsible for supporting the contractual needs of the program, including managing referrals to community resources and multi-agency reporting as required.
- Responsible for developing strategies to have a timely response to cross-departmental referrals within JPNDC to develop Individualized household Plans and support administration functions for designated individuals, assisting them in identifying choices, preferences, and strengths through a self-determination planning process where the individual, family members and/or personal advocate are in charge.
- Identify resident's needs (medical, social, emotional, financial, etc.) and connect them with relevant local and regional resources.
- Provide case management services, including assessment, planning, implementation, coordination, monitoring, and evaluation of services.
- Serve as a neutral mediator between residents and management to help resolve issues such as non-payment, housekeeping concerns, and other disputes, with a focus on tenant preservation.
- Complete required trainings as assigned.
- Design and implement programs that help reduce rent arrears, improve lease compliance, and promote positive resident relationships
- Create and foster a network of local community-based groups and social service agencies and facilitate resident's access to programs and services.
- Organize activities, support groups, and social events to foster a sense of community and encourage resident engagement. Act as an advocate for residents' needs and help resolve issues, including providing staff education on resident's rights.
- Develop and implement resident programs, special events, and initiatives to enhance residents' quality of life.
- Maintain accurate and confidential records of resident interactions and program activities.
- Provide families with high-level and ongoing support in navigating referral processes.
- Other duties as assigned by supervisor.
Qualifications
Required:
- Bachelor's degree in Social Work or a related Human Services Degree.
- Minimum 3 years direct service experience (1+ yr supervisory) in one or more of the following: working with families of underprivileged socio-economic backgrounds, working with multicultural populations meeting diverse family needs, stabilizing challenging circumstances, & supporting families who have experienced trauma.
- Strong understanding of LIHTC, HUD and Fair Housing regulations related to affordable housing.
- Ability to nurture relationships across services and resources agencies.
- Sensitivity to and experience with handling confidential information and interactions, interacting in a culturally appropriate and responsive manner with a wide range of individuals.
- Able to use own vehicle for work daily.
Preferred
- LSW or mental and/or behavioral health curriculum training and licensing preferred.
- Master's Degree, LSW in mental and/or behavioral health or related certification.
- Bilingual (English/Spanish/Haitian/Cape Verdean Creole).
- 5+ years in Human Services, or related field.
