Overview
We are looking for a dedicated and empathetic Licensed Vocational Nurse (LVN) to join our outpatient hospice care team. The ideal candidate will deliver compassionate, high-quality nursing care to patients with life-limiting illnesses in both outpatient and home settings. This position requires strong clinical skills, emotional sensitivity, and a solid understanding of hospice principles. The LVN will work closely with an interdisciplinary team to provide comfort-focused, holistic care that supports patients and their families throughout the end-of-life journey.
Duties (include, but not limited to):
- Conduct physical examinations and assess patients' health conditions.
- Implement infection control protocols to maintain a safe environment for patients and staff.
- Ensure proper patient care and education.
- Perform triage duties to prioritize patient needs effectively.
- Manage administrative tasks within the medical office, including maintaining patient records and scheduling appointments.
- Execute suture procedures as needed, ensuring proper technique and care.
- Utilize various systems for documentation and patient management efficiently.
- Deliver primary care services, including monitoring vital signs and administering medications.
Requirements
- Possess a current and active Licensed Vocational Nurse (LVN) license in the state of CA
- At least one year of LVN experience in hospice, palliative care, or outpatient settings is preferred.
- Understanding of hospice regulations, documentation standards, and Medicare guidelines.
- Knowledgeable in symptom control, comfort-focused interventions, and the emotional aspects of end-of-life care.
- Excellent interpersonal and communication skills with the ability to work effectively within an interdisciplinary hospice team.
- Committed to providing compassionate, respectful care to patients from diverse backgrounds.
- Capable of safely lifting, repositioning, and assisting patients with personal care when necessary.
- Open to traveling to patient residences or care facilities for scheduled visits.
