We are looking for an experienced and motivated per diem Registered Nurse (RN) to join our growing team in East Haven, CT
About Us: Utopia Home Care is a Medicare certified, state licensed, medical home care agency that provides in-home nursing, therapy, and home health aide services throughout Connecticut. We have been family owned and operated since 1983 and currently serve over 1400 clients throughout the state
Behavioral Health Registered Nurse (RN) Qualifications / Requirements:
- Must have an RN or BSN certification and possess the original document of certification from a state approved training provider.
- Must be able to provide original documents for CT Driver's License or CT State ID and original Social Security Card.
- Must be able to provide documentation of a physical and PPD taken within the past 12 months OR be willing to obtain a physical and PPD test quickly.
We are especially looking for:
- Behavioral Health experience
- At least 1 year of experience in a home care setting
- Bilingual in English and Spanish
No home care experience? That's ok We have an assigned nurse educator who provides training as part of the on-boarding process.
Registered Nurse (RN) Job Duties: Registered Nurses (RN) Case Managers oversee all medical services our clients receive which includes nursing, physical / occupational therapy, and home health aides. This begins with a comprehensive clinical assessment and creation of a specialized care plan, consistent with recommendations from the hospital, skilled nursing facility, and primary care physician. Direct nursing services include periodic health assessments, teaching, skilled intervention, care coordination, and medication management. All of this takes place in the client's home
