Salary and Benefits

  • Base salary: $140,000 annually
  • Annual bonus potential: Up to $10,000 in the first year (increases with partnership opportunities)

  • Vision insurance: $3.10/month for employee-only coverage with $180 frame allowance every 24 months
  • Health Savings Account: Employer contributes $500/year for individuals or $1,000/year for families
  • Short-term disability: Employer-paid; 60% of weekly salary up to $2,000/week
  • Long-term disability: Employer-paid; 60% of monthly salary up to $15,000/month
  • Life insurance and AD&D: Employer-paid; coverage equal to 1x annual salary up to $250,000

Responsibilities

  • Full-time, Monday–Friday schedule
  • Clinic hours: 7:30 AM – 5:00 PM (last patient scheduled at 4:15 PM)
  • Outpatient primary care setting (100% outpatient)
  • Patient volume: 18 patients per day
  • Small patient panel: Maximum of 450 patients
  • Focus on senior patients with 3–5 chronic conditions
  • Provide preventive, holistic, and value-based care
  • Supported by a dedicated team, including your own Medical Assistant and Scheduler
  • Administrative time included in the daily schedule
  • No nights, no weekends, and no hospital rotations
  • Access to in-house consulting specialists including cardiology, podiatry, acupuncture, tai chi, yoga, and nutrition services
  • Door-to-door transportation available for patients

Requirements

  • Board Certified Family Nurse Practitioner or Geriatric Nurse Practitioner
  • Active and unrestricted Nurse Practitioner license in Kentucky
  • At least 2 years of primary care outpatient experience required
  • Passion for serving the senior population with a value-based care model
