POSITION: Physical Therapist

SUPERVISOR:       Outpatient/Inpatient Senior Clinical Supervisor, Director of Clinical Rehabilitation Services

SHIFT: Part Time, days

HOURLY RATE:      $41.00 -$44.00 per hour, commensurate with experience

Engages in timely, creative, and effective use of assistants, aides, and volunteers.

Consistently displays a positive, caring, and responsive attitude towards employees and any individuals who interact with the facility.

Demonstrates knowledge of, and rational for, physical therapy techniques; modalities; equipment use; and exercise programs.

Establish both long- and short-term goals and a thorough treat­ment care plan by which the patient/resident can progress towards goals and modify care plan appropriately/timely when needed.

If appropriate for patient/resident needs, collaborates with patient's/resident's families; significant others; and staff prior to patient/resident discharge in order to formulate an appropriate discharge or follow-up plan.  Recommendations may include, but not limited to, continuation or discontinuation of services; implementation of home programs; recommend appropriate adaptat­ions/equipment for use in home environment; and/or recom­mends community resources.

When appropriate, effectively instructs patients/residents/ families in techniques and home programs by structuring content/   ­presentation to appropriately reflect patients'/residents'/family member's age, communication abilities, culture and educational level, listens to their concerns, and answers questions to the best of therapist's ability.

Organizes schedule of patient/resident treatments taking into account the work demands of other employees/departments.

Instructs patient/resident/family in the application, care, and precautions of orthosis/prosthetics as ordered by physician at Orthotic/Prosthetic Conference.

Provides objective, accurate, and timely documentation according to policy on assigned patients/residents via initial evaluations, progress notes, care plans, team conference reports, FIM sheets, MDS, charge/attendance checklist, discharge summary, and all other miscellaneous facility forms as necessary.

Completes documentation for insurance requests in a timely fashion.

Leaves a written, clear treatment plan (coverage notes) on all assigned patients/residents when scheduled out of the department.  May use phone to address above as appropriate.

Demonstrates initiative to assist with the evaluation and devel­opment of documentation systems, forms, and charting mechanisms to facilitate efficient and effective documentation.

Recommends equipment and/or supplies when inventory demonstrates the need to reorder.

Attend one or more continuing educational programs and/or equiva­lent annually and provide staff in-service.

Provide at least one in-service per year for the department/facil­ity staff as well as review current literature review on clinical topics.

Participate in program development/revision with the facility.

Serve as a clinical instructor (CI) to supervise and teach a student affiliate following the facility clinical education manual and academic student manual.  Create a positive and constructive learning environment with the student. It is expected available CI's mentor at least one student per year or more at the CI's request.

Maintains level of productivity as established for the depart­ment.

Participates in department QA programs, including collecting of data, analysis, and documentation review.

Will travel to contract sites as needed.

Assists supervisor in developing new innovative ideas for QA studies related to patient/resident care related problems and department operations.

Performs duties according to the highest ethical and moral standards and in adherence with the facility's corporate compliance plan and all applicable governing laws.

Performs all other duties as assigned by supervisor to ensure that the department functions and operates as smoothly and efficiently as possible.

 
Qualifications

Degree in Physical Therapy from an accredited program.

Staff: Basic entry level - experienced achieved through aca­demic curriculum and clinicals and/or up to three (3) years work experience.

Possession of, or eligible for, a valid Physical Therapy license issued by the state of New York, CPR certification.  If sitting for licen­sure examination, there is a condition of passing the licensing exam within one year after employment and during the interim possess a New York State temporary permit.
