ROLE SUMMARY: Join our Veterans Rally Point program as an SSG Fox Grant Program Manager and make a meaningful difference in the lives of those who served. This rewarding leadership role offers the opportunity to provide oversight, mentor a dedicated team, and build strong community partnerships while ensuring compliance with SSG Fox grant requirements.

We're looking for a compassionate leader with experience supervising staff and a deep understanding of the unique challenges faced by Service Members, Veterans, and their families is advantageous. If you're passionate about veteran advocacy and want to lead impactful work in a supportive environment, we want to hear from you. Military service or military-connected background preferred but not required.

Key Responsibilities:

  • Ensure compliance with SSG Fox project milestones, outcomes, data collection, and federal guidelines
  • Lead and mentor a team of Case Managers through interviewing, onboarding, training, and supervision
  • Establish criteria and processes for client intakes, assessments, case management, and individual service plans
  • Coordinate warm handoffs to VA facilities and collaborate with VA Suicide Prevention Coordinators
  • Develop and implement formal policies for outpatient behavioral health operations
  • Conduct quarterly auditing processes for behavioral health staff and work toward CARF accreditation
  • Establish and monitor referral processes for basic needs services
  • Manage data collection, analysis, and reporting in coordination with Director of Grants
  • Serve on SERAC/DMHAS Eastern CT Suicide Prevention Regional Advisory Board and CT Governors Challenge
  • Conduct suicide assessments of walk-ins and engage in active treatment planning and care coordination
  • Build community partnerships and engage in outreach with local behavioral health and medical practices
  • Provide trauma-informed assessment and service coordination for at-risk populations

SKILLS AND QUALIFICATIONS:

  • Master's in Social Work or related field preferred
  • LCSW, LPC, LMFT, or LADC preferred
  • Experience in supervisory and/or leadership role
  • Demonstrated proficiency in policy formation and implementation
  • Community outreach experience
  • Experience diagnosing and treating co-occurring disorders and substance use disorders
  • Trained in trauma-informed practices and procedures
  • Experience working with veterans and at-risk patient populations highly preferred
  • Experience with EHR systems
  • Proficiency with Microsoft Office Suite and ability to manage multiple projects simultaneously
  • Valid driver's license with proof of insurance; willingness to use personal vehicle
  • Ability to maintain composure under pressure and handle confidential matters professionally
  • Military service or military-connected background preferred

LOCATION: Norwich, CT (in-office position)

SALARY: $75,000 - $83,000

Job Type: Full-time

Pay: $75, $86,000.00 per year
