Victory Recovery Partners, an outpatient addiction-recovery center, is currently looking for a Certified Recovery Peer Advocate (CRPA) to join our team in our Hempstead, NY location. Peer support services target recovery outcomes such as improved health and wellness; an increased sense of self-efficacy or empowerment; and increased success and satisfaction in a range of community settings such as work, home and school, instead of merely focusing on symptom reduction. Through a combination of lived experience and professional training, peers can provide an array of face-to-face peer support services with a patient.
This is a full-time position, and your schedule will include 2 shifts per week of 9am-5pm, and 3 shifts of 12-8pm per week.
Duties/Responsibilities:
- Engage an individual to consider entering treatment.
- Encourage a patient to attend treatment their current treatment schedule and/or encourage the patient to make use of other services offered by Victory Recovery Partners or other outside healthcare services.
- Support and encourage an individual in continuing care services post-discharge.
- Raise awareness of existing social and other support services.
- Practice in collaboration with counselors, MAT providers and psychiatric providers to manage patient care.
- Assess the multidimensional needs of patients and assists in developing and implementing care plans.
- Function as a patient advocate, by participating in clinical meetings, case conferences and informal discussion with prescribers and other clinical staff.
- Provide written reports and care coordination as needed.
- Attend and participates in continuing educational programs, seminars, etc., designated to maintain professional competence.
- Utilize an Electronic Health Record to document all patient care, accurately and timely - in compliance with Victory Recovery Partners policies and procedures.
Qualifications & Requirements:
- High School Diploma or equivalent.
- Spanish Speaking - Required
- Certified Peer Recovery Advocate Certification required.
- Knowledgeable in the field of addiction medicine, social work.
- Demonstrated interest in helping people.
- Ability to empathize with patients.
- Effective at functioning in a fast-paced environment
- Skilled in communicating orally and in writing.
- Excellent interpersonal skills; able to work effectively and non-judgmentally with diverse populations and communities.
- Dependable, reliable, and trustworthy.
Job Type: Full-time
Pay: $21.50 per hour
