Job Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team in the Dallas area. This nurse will be responsible for a case load to include Dallas, Carrolton, and north. This role is essential in coordinating patient care, ensuring quality outcomes, and facilitating effective communication among healthcare providers, patients, and families. The ideal candidate will possess strong clinical skills and a thorough understanding of medical terminology, discharge planning, and case management processes.
Duties
- Assess patient needs and develop individualized care plans based on comprehensive evaluations.
- Ensure compliance with HIPAA regulations while managing sensitive medical records.
- Collaborate with multidisciplinary teams to facilitate effective discharge planning and follow-up care.
- Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
- Educate patients and families about health conditions, treatment options, and available resources.
- Maintain accurate documentation of patient interactions and care provided in accordance with medical standards.
Qualifications
- Current Registered Nurse (RN) license in the state of practice.
- Experience in hospice and terminally ill patients.
- Strong understanding of medical terminology and case management principles.
- Excellent communication skills with the ability to work collaboratively in a team environment.
- Proven ability to manage multiple cases effectively while maintaining high standards of patient care.
- Commitment to ongoing professional development and adherence to nursing best practices.
