How You'll Make an Impact
- Build trusting, compassionate relationships with patients and families
- Empower caregivers through education, guidance, and emotional support
- Collaborate with the interdisciplinary team to provide comfort, peace, and dignity
- Coordinate and oversee all aspects of in-home patient care
Key Responsibilities
- Develop, implement, and evaluate individualized care plans
- Perform head-to-toe assessments and monitor vital signs
- Administer medications, injections, IV therapy, and tube feedings
- Manage catheters and other medical devices
- Educate patients and families on care needs and expectations
- Supervise and support Home Health Aides (HHAs)
- Complete accurate, timely documentation
- Communicate effectively with the Clinical Manager, physicians, and care team
- Participate in the on-call rotation (typically once per month, including evenings/weekends as needed)
- Perform other related duties as assigned
What You'll Need
- Current Registered Nurse (RN) license in the state of Texas
- Minimum of 2 years of RN experience (hospice experience preferred)
- Current CPR and BCLS certification
- Valid driver's license and reliable transportation
- Strong communication and interpersonal skills
- Excellent organization and attention to detail
- Comfort with technology and timely clinical documentation
