POSITION SUMMARY:
Under the Direction of the Shelter Program Manager, the Shelter Case Manager works with an active caseload of 17-20 families; designs individual plans in collaboration with the family aimed at moving them out of homelessness. The relationship between the case manager and the client is one of advocacy and collaboration. The case manager meets weekly with the family, provides mediation and helps to develop a network of supports both on and off site.
EDUCATION AND EXPERIENCE:
- Bachelor's Degree in Social Service field
- Minimum one year experience working with case management in public or non-profit setting.
- Bilingual English/Spanish preferred.
Essential Duties and Responsibilities: The following statements are intended to describe the general nature and scope of work being performed by this position. This is not a complete listing of all responsibilities, duties and/or skills required. Other duties may be assigned.
- Participate in all group intakes and individual intakes as needed.
- Following the initial interview, schedule weekly meetings with residents to create a plan of action that will lead them out of homelessness and monitor this plan for effectiveness.
- Keep records of meetings and actions taken.
- Evaluate new and existing client's needs to determine their immediate needs, functional capabilities and assistance needed to stabilize housing, healthcare and finances.
- Make client assessments in current settings. Attend the resident's house meeting first Monday of each month.
- Provide follow-up and re-assessment to maintain the most current information on families needs at least once a week or more frequently if necessary. Intervene in acute situations to meet essential needs when usual supportive service arrangements are not available.
- Arrange for timely, appropriate delivery of all services needed for families to maintain the highest possible level of independence.
- Advocate for families when necessary to obtain needed service.
- Ongoing coordination of all support services to assigned families.
- Collaborate with team members from other disciplines to determine the best course of service delivery for each family.
- Participate in the development of a treatment plan for each family.
- Report all instances of suspected child abuse to appropriate agencies.
- Assure translation services when necessary.
- Attend and participate in all required agency meetings: staff meetings, in-service training, resident house meetings and weekly supervision.
- Participate in and pass annual CPR course provided by agency.
- Follow all agency protocols regarding safety and professional behavior; adhere to all COVID Protocols and Procedures
- Keep the staff apprised of the progress of each resident family.
- Attend all staff meetings and give updates on resident progress.
- Meet with the Program Director and staff on a weekly basis.
- Maintain current knowledge of services and resources available for homeless families..
- Ability to maintain the confidentiality of sensitive information.
- Maintain up-to-date written documentation of family's problems, needs, interactions, interventions and responses in each family's chart.
- Follow standard charting protocols for documentation in families' charts.
- Maintain accurate statistical information of families seen and services provided.
- Provide statistical and other reports as required and in a timely manner.
- Establish working relationships with all public agencies concerning housing, job training, job placement, education and addiction counseling.
- Other duties in keeping with the scope and nature of the position.

- Maintain accurate statistical information of families seen and services provided.
- Provide statistical and other reports as required in a timely manner
- Strong computer skills
· Be familiar with all Shelter policies in order to respectfully serve the Shelter population and represent the Shelter effectively when working with public agencies.

While performing the duties of this job, the employee is frequently required to maintain mobility to work in an office setting and operate a variety of equipment, operate computers and other office equipment, move about the office, attend meetings, and communicate effectively.
Tasks require the ability to exert light to moderate physical effort which involves lifting, carrying, pushing and/or pulling of objects and materials of light weight (5-15 pounds).
