Sentara Health is seeking to hire a qualified individual to join our team as a Social Worker.

Position Status: Full Time

Shift: Monday - Friday, 8:30 am - 5:00 pm

Position Location: Sentara PACE, Norfolk, VA (travel in Hampton Roads required)

Overview
Acts as an Advocate for the participant and as a representative of the team. Acts as a liaison between participants, families and staff. Assist participants in maintaining and achieving a maximum psychosocial function level. Maintain appropriate records to meet state, local and federal regulations. Knowledge of PACE, DMAS & CMS regulations.

Make home visits to conduct initial, three month and five-month assessments as required by regulations & as needed. Act as a liaison between participant and other interest agencies. Develop and maintain participant care plans. Assure participants are aware of their rights and responsibilities. Participate on intake and assessment team as well as the multidisciplinary team. Serve as a resource to staff, participants and families. Provide counseling to participants and families as needed. Maintain charting/documentation within state, federal and local regulations time frame. Monitors and maintains flow of information, both verbal and written, through department and interdepartmentally. Screens admission reports for work list on payers who will require clinical review. Maintains files in orderly manner that facilitates rapid retrieval. Assists in documentation of data to trend denials information and provide feedback to clinical team, Contracting, Patient Accounting, and Revenue Management. Places clinical documentation in appropriate format/software program for distribution to the third party payer. Maintains HIPPA requirements for transmission clinical data to payers and within the department. Collaborates with Insurance Verification and Registration staff to ensure accuracy of payer information. Places necessary information into HBOC and other software programs, as applicable, to document date, time of transmission, data provided, recipient, and other important information related to payment and certification of stay, impending or actual denial issues. Documents all patient contact and activities related to the patient's case. Mails all letters and certified notifications to meet requirements. Updates and maintains Tracking Spreadsheet for Pharmacy and Provider restrictions.

  • Driver's License
