Role:
The CCRN SUD (Substance Use Disorder) Care Program Coordinator is a Complex Care Registered Nurse (CCRN) for SUD Care responsible for providing high-quality, patient-centered care for individuals with substance use disorders, with 75% of time dedicated to direct patient care. Reporting to the Clinical Nurse Manager, the Coordinator also performs administrative duties, ensuring compliance, quality monitoring, and effective coordination of Harbor Health's SUD services across all sites. This role focuses on delivering evidence-based treatment in collaboration with an interdisciplinary care team.
Responsibilities:
Patient-Facing Responsibilities (75%):
Conducts initial assessments and intakes, gathering histories including social, medical, mental health, substance use, and medication history.Triages and provision of low-barrier access for all phone and walk in inquires for treatment.Reviews assessments with the SUD clinical team to determine appropriate care plans. Manages patient care plans, performs crisis intervention and management of referrals.Collaborates and communicates with the SUD team, attending meetings as required. Provides ongoing education, support, and service coordination for patients. Serves as liaison and provides education and support for primary care teams and community partners such as hospitals, VNA, labs or other medical offices, outreach and teams. Assists in creating clinical forms and ensures meticulous documentation.Collects pertinent data and specimens, reviews commonly found results with patients, taking clinical actions per protocol. Administers immunizations as requested by providers or per protocol and accurately documents in the EMR.Coordinates with the pharmacy and SUD Care team on medication orders, medication administration, prescriptions, and refills. Counsels patients on relapse prevention and pharmacotherapy and provides referrals and resources for non-pharmacotherapy as indicated. Provides follow-up care, scheduling clinical visits and phone support. Engages in primary care and addiction-related projects. Collects and submits required data for the Bureau of Substance Abuse Services. Assists with phone triage, prescription refills, follow-up test results, and managing medical issues. Assists providers in the provision of care: procedures, injections, EKG's, referrals, prior authorization, specialty pharmacy, security and documentation of controlled substances. Performs all nursing duties in accordance with Massachusetts Nursing Practice Act, while adhering to Harbor Health policies and procedures. Follows HHSI policy when disposing of biohazardous materials, labeling and handling all materials.Administrative Responsibilities (25%):
Collaborates with nursing leadership and the Director of SUD Care on compliance activities, including policy and protocol development, maintaining accurate documentation, monitoring treatment outcomes, and preparing reports for regulatory agencies. Participates in program planning and quality improvement initiatives to enhance service delivery. Coordinates patient referrals to internal and external providers, ensuring continuity of care. Supports training and education for team members involved in SUD services, promoting adherence to evidence-based care standards. Maintains accurate patient records in the electronic health record (EHR) system, ensuring all care activities are documented appropriately. Attends team meetings, case conferences, and community outreach events to support Harbor Health's SUD program goals.Requirements:
Successful completion of a nursing program (RN), BSN preferred Current Registered Nurse (RN) license in Massachusetts requiredCertification as a Certified Addictions Registered Nurse (CARN) or commitment to obtain within 6 months of hireCurrent BLS (Basic Life Support for Healthcare Providers) Certification Minimum of 2 or more years of clinical experience in substance use disorder treatment, behavioral health, or addiction medicineProven strong leadership, critical thinking, problem-solving, administrative and organizational skills to meet competing priorities Experience with medication-assisted treatment (MAT) programs and withdrawal management preferredExperience in outpatient or community health settings, with a focus on cultural sensitivity and the ability to work comfortably with diverse economical, racial, multi-linguistic and ethnic populations, is highly desirableComprehensive understanding of substance use disorders, MAT protocols, and withdrawal management. Excellent communication and interpersonal skills, with the ability to build trust with patients and collaborate effectively with care teamsBasic to Intermediate knowledge of Microsoft Office 365 including Word, Excel, Outlook and Teams, OCHIN/EPIC Electronic Medical Record (EMR) experience preferredExperience or training in de-escalation preferredRequires knowledge of processes and community resources and agencies relative to health, protective services, crisis intervention, disability, etc. to aid in adjusting or tapering controlled substancesWillingness to train and orient new personnelStrong patient advocate and an anti-stigma change agentAll qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability or veteran status.

The salary range and/or hourly rate listed is a good faith determination of base compensation that may be offered to a successful applicant for this position at the time of this job advertisement. When determining actual base salary and/or rate, several factors may be considered as applicable (e.g. location, years of relevant experience, education, training, and other factors as permissible by law).
8:00 AM - 4:30 PM Monday - Friday, subject to change based on program/clinic needs.
